Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F05611-03-Q-0718, in accordance with FAR Parts 12 & 13. The North American Industrial Classification System (NAICS) number is 811490 and the business size standard is 500 employees. The proposed acquisition is reserved for small business. LINE ITEM 0001: Provide Equipment Maintenance services for the USAF Academy Fitness Center, 1 Dec 02 through 30 Sep 03, ten months. LINE ITEM 0002: Option year one. LINE ITEM 0003: Option year two. LINE ITEM 0004: Option year three. The FOB for this RFQ is Destination. Quotations received with other than FOB Destination pricing will not be considered. Your proposal should be provided as a monthly amount and a yearly-extended amount for the base and option years. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Comercial Items, applies to this acquisition and on addenda applies. The provisions at FAR 52.212-2, Evaluation Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose acceptable offer conforming to the Statement of Work, will be most advantageous to the government, price and other factors considered. Factors considered for award are price and past performance. Price and past performance will be of equal value. Identify past or current contracts (including Federal, State, and local government and private) for efforts similar to the Government requirement, within the last three years. Past performance information should include: company name, address, phone/fax number, POC/e-mail address, and contact number if applicable. The Clause 52.212-3, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this acquisition. Additional FAR clauses cited in the clause applicable to this acquisition are: (b) 12,13,14,15,24, (c) 1,2,3,4. Reference (c) 1, of 52.212-5, Wage Determination 1994-2079, Rev 23 applies. Reference (c) 2, Wage equivalence, General Maintenance worker, GS-08, 13.94 per hour. The Clause 52.217-8, Option to Extend Services applies to this acquisition. The Clause 52.217-9, Option to Extend the Terms of the Contract applies to this acquisition. The Clause 52.232-18, Availability of Funds applies to this acquisition. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Status of Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this acquisition with the inclusion of the following DFARS: 252.204-7004, Required Central Contractor Registration. STATEMENT OF WORK (SOW) PREVENTATIVE MAINTENANCE CARDIOVASCULAR AND WEIGHT EQUIPMENT FOR USAF ACADEMY, CO 1. Description of Work a. Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to inspect and repair cardiovascular and weight equipment in the Fitness Center in accordance with (IAW) the SOW. b. The Fitness Center Director or Operations Director will approve all repairs prior to beginning work. All replacement parts required for maintaining equipment will be ordered within 72 hours from time of approval. 2. Personnel a. Contract Manager: The individual is responsible for the performance of the repair work. If this individual is not available, an Assistant Contract Manger is responsible. The name(s) of the individual(s) will be provided to the Fitness Center Director or the designee at the time of contract award. b. Employees: The government is authorized to restrict the employment under this contract of any contractor employee, or representative contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of these facilities or its patrons. c. The contractor will provide documentation substantiating their service technicians are qualified to service and maintain this equipment identified in the SOW. 3. Quality Control a. The contractor will ensure that all work performed under this SOW meets the quality standards established in paragraph 7. b. Any deviations to paragraph 7 must be in the scope of the SOW and approved by either the Fitness Center Director or designee before repair. 4. Hours of Operation a. The contractor shall schedule all work to be done on equipment between the hours of 8:00-5:00 PM Monday-Friday, excluding federal holidays. They are as follows: New Year?s Day, Martin Luther King Day, President?s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran?s Day, Thanksgiving, and Christmas. b. The contractor shall coordinate all preventive maintenance inspections with the Fitness Center Director or designee, which will be performed in accordance with paragraph 7a. 5. Government Furnished Supplies and Services a. An individual from the Fitness Center is to be on site for scheduled service calls. b. The Fitness Center Director or designee is required to sign all service reports, verifying the satisfactory completion of the service(s) performed. c. The Fitness Center will have service manuals on-site for the contractor to reference. Any manuals required in addition to the manufacturer?s manual shall be provided by the contractor at his or her expense. 6. Contractor Furnished Parts and Materials a. Replacement parts: The contractor shall provide all parts unless the part(s) is available by the Fitness Center. The Fitness Center Director or designee will approve the contractor-supplied part(s) in accordance with paragraph 7f. b. All replacement parts will be new factory warranty parts, unless replacement manufacturer rebuilt parts are approved by the Fitness Center Director or designee. All parts replaced remain property of the fitness center and must be made available at the completion of the repair. 7. Specific Tasks a. The contractor will provide scheduled preventive maintenance inspections (PMI?s) and on-site unscheduled service calls in accordance with the manufacturer specifications. b. PMI?s shall be accomplished within the prescribed timeframe as recommended by the manufacturer. They consist of the care and servicing of the equipment for the purpose of retaining the equipment in a serviceable condition in accordance with the namufacturere?s maintenance specifications. The specifications are listed in their technical literature. This inspection includes checking for proper operation, detection, and correction of incipient failures, either before they occur or before they develop into major problems. This includes adjustments, calibration, interior cleaning, and replacement of worn to deteriorated parts. c. Upon arrival the contractor?s service technician will work continuously until the repair(s) is completed per the manufacturer specifications. The only exception is a delay due to waiting for replacement parts. d. The contractor will respond to unscheduled service calls within 72 hours of notification. e. The contractor?s service technician will prepare a service report for each service call (scheduled or unscheduled) and obtain a signature from the Fitness Center Director or designee to certify that the services were completed and accepted. f. Before replacing parts on any piece of equipment, the contractor will provide to the Fitness Center Director or the designee a list of parts required, as well as the costs of parts. No part will be replaced without approval. The government reserves the right to purchase and provide the contractor with the parts required to bring any piece of equipment up to standard. g. The contractor will obtain estimates for parts from the supplier. The contractor will then forward the information to the Fitness Center Director or designee for purchase approval or the parts will be obtained by the fitness center and provided to the contractor. 8. Uptime is defined as the time the equipment is performing according to the manufacturer?s specifications. Downtime is defined as the time when the system is deemed not available due to breakdown, ongoing corrective maintenance, or failure to perform according to the manufacturer?s specification. 10.
