Description
(I) This is a combined synopsis/solicitation for commercial services prepared in the format in the FAR, Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation. NO OTHER DOCUMENTS WILL BE ISSUED. (II) This solicitation is issued as Request for Quotation (RFQ) KH1-03039. (III) This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 09, dated 09/30/2002. (IV) This solicitation is set aside 100% for small business. The NAICS code is 923110. (V) A - Service to be Performed: In conjunction with the Chaplain Program Manager, the contractor shall coordinate all program components in the "Residential Life Connections Pre-release Program" a multi-faith program designed to improve discipline and lower recidivism; assure all inmate participants are receiving instruction from qualified contractors and mentoring from qualified volunteers; maintain and disbribute the weekly schedule of activities; assist the Program Manager in developing program components, bringing in speakers and leading group activities. The contractor shall be expected to commit 30 hours per week to the program. The schedule may include some weekend and/or evening hours depending on program needs. The schedule and any changes will be negotiated between the contractor and Program Manager. The contractor shall be knowledgeable of program goals; will work with institution staff and community groups. The contractor must possess at a minimum a Bachelor of Arts degree from an accredited college or university, with a major in religious studies, religious education, human relations, public administration or a related field. The contractor shall have experience and demonstrable success in managing multiple tasks, interpersonal communication and leadership skills. The contract awardee must successfully complete a background investigation, urinalysis, and a 4-hour orientation program prior to assuming duties. (VI) Evaluation Factors: The contract will be awarded based on education and experience, past performance and price (in descending order, with price being the least important). Offers must include verifiable experience, references and other information pertinent to the solicitation. The contract may be awarded without discussions, so offerors are encouraged to submit their best offers initially. (VII) Performance Period: 12/01/2002 thru 09/30/2003. THIS IS NOT A PERSONNEL APPOINTMENT. The award of the contract does not constitute an employee/employer relationship. The contractor will not be supervised, but will be monitored to determine if services rendered are in compliance with the contract terms and conditions. Payment will be made upon receipt of an invoice, and after verification that contract terms and conditions have been met. (VIII) FAR provision 52.212-1, Instuctions to Offerors-Commercial Items (Oct 2000), is referenced. At paragraph H, Multiple Awards, the Contracting Officer has determined multiple contracts are NOT applicable to this procurement action, and the paragraph is deleted in its entirety. (IX) FAR provision 52.212-3, Offeror Representations and Certifications-Commerical items will be completed prior to award. (X) FAR provision 52.232-33, Mandatory Information for Electronic Funds Trasfer Payment applies to this solicitation. In addition, contractor MUST provide federal tax identification or Social Security number PRIOR to award. (XI) Offers will NOT be accepted by fax. Offers must be received in the Contracting Office no later than 2:00p.m., central time, October 21, 2003. Envelopes must be clearly marked with the solicitation number. THIS IS NOT A SEALED BID. Late offers will not be rejected, unopened.
