Description

This is a combined synopsis / solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; quotes are being requested and A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number / RFQ N00244-03-T-0141 applies; an electronic copy of the RFQ can be accessed at http://www.neco.navy.mil. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-04 and DFAR Change Notices effective through 01/29/02. The standard industrial code is 3826 (NAICS 334516) and the business size standard is 500 employees. This requirement is for a fixed priced contract for a maintenance contract on laboratory equipment. This contract is for a base year and two option years. There are two pieces of equipment to be repaired and maintained one (1) each spectrophotometer ICP, mdl JY-2000 and one (1) each modular cooling system, mdl Lytron MCS. Both pieces of equipment are located in San Diego CA. The statement of work is as follows: General Requirements - The effort required hereunder should be performed in accordance with this statement of work and in acco rdance with all other terms and conditions set forth herein. The Contractor shall provide services, with a two day response time (2RT) for routine or emergency service of one (1) each inductively coupled plasma spectrophotometer (JY-2000) and one (1) each modular cooling system (Lytron - MCS) to ensure dependable and reliable operation of equipment. This equipment is located in the Health Surveillance Laboratory (chemistry lab), Naval Station, Department at the Navy Environmental and Preventive Medicine Unit #5 (NEPMU-5), San Diego CA. The Contractor shall provide all services, materials and equipment necessary for the maintenance of the inductively coupled plasma spectrometer (ICP), mdl JY-2000 with the Lytron modular cooling system. The scope of work performed under these specifications includes the furnishing of all labor, equipment, materials, and parts to perform all s ervices for the preventive maintenance, repair and inspection of all equipment listed to assure continued operation at their designed efficiency and capacity. Inspection shall include cleaning, lubrication, verifying electrical calibration, mechanical and electrical adjustments and installation of all necessary parts. The Contractor shall perform the following functions: 1) Perform all maintenance to industry standards. 2) Insure that only fully qualified field engineers and technicians, who have gone through original equipment manufacturer (OEM) or compatible third party service schools for the above mentioned equipment, shall be employed in the performance of any and all work performed under this contract. The highest standard of professional capability and electrical / mechanical workmanship is to be maintained throughout the life of this contract. 3) Make repairs to t he extent necessary (as determined by inspection tests or disassembly) to ensure a functional system that will efficiently serve its intended purpose. 4) Correct inoperable condition in a timely manner including service response time of no more than 48 hours after telephone notification, to a daytime eight (8) hour 365 day monitored contract telephone number to be provided by the Contractor, to the Consolidated Industrial Hygiene Laboratory upon contract award. 5) Check, clean, adjust, and calibrate operation and scanning functions regularly and according to written schedule. 6) Replace worn, failed or doubtful components and parts including worn or frayed electrical wire or power cords. Where statistical data on similar equipment indicated failure point is approaching for any component, said component will be repaired or replaced in advance to prevent system downtime / fai lure. 7) Provide only the work necessary to restore the equipment to a serviceable / operating condition by adjustments, replacement parts, or minor repairs when it is determined that extensive repairs and parts replacement are not necessary. 8) Install upgraded components and make system modification and improvements on a regular basis that is recommended by the manufacturer. Equipment improvements / modifications shall be made only on written approval and direction of the Consolidated Industrial Hygiene Laboratory of NEPMU-5. 9) Notify the Consolidated Industrial Hygiene Laboratory of NEPMU-5 immediately upon receipt of OEM or replacement parts / equipment safety recall notices. 10) Ensure that original design and functional capabilities will not be changed, modified, or altered unless the Consolidated Industrial Hygiene Laboratory of NEPMU-5 authorizes such changes. 11) Provide suitable modern recommended repair equipment / tools required for the satisfactor5y execution of all repairs made. 12) Furnish manufacturer OEM approved lubricants and lubricate wear points within the equipment. 13) Ensure that all work areas are properly cleaned of all repair / maintenance work residual immediately after completion of service call. 14) Extend the Government all commercial warranties on replacement parts, consistent with standard industry practices. 15) Maintain and adequate spare parts inventory on parts with a history of high failure / replacement rates in an established service facility located geographically to permit a 48 hour service call response arrival at Consolidated Industrial Hygiene Laboratory of NEPMU-5. Special Conditions - Title to Equipment: The Contractor shall not assume possession or control of any part of the equipment. The Government retains ownership to title thereof. Liability: The Contractor shall not be liable for any loss, damage, or delay due to any cause beyond his reasonable control including, but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of God. Utilities: The Contractor may use Government utilities (electrical power, compressed gases, argon, air, and nitrogen) that are available and required for any service performed under this contract. Contractor electrical equipment must b approved by the building engineer to ensure compatibility with Navy Environmental ad Preventive Medicine Unit #5 electrical wiring and equipment. Access to Equipment: Contractor personnel shall first report to the Chemistry Teal Leader (CIHL) or his designated representative, prior to commencement of any work under this contract. The Contractor will be provided reasonable access to all equipment that is to be serviced and utility outlets required to the service. The Contractor will be free to start and stop all primary equipment incidentals to the operation of the maintained equipment after permission is received from the Chemistry Team Leader (CIHL) or his designated representative, responsible for such equipment. Government Personnel - Consolidated Industrial Hygiene Laboratory of NEPMU-5 and Navy Environmental and Preventive Medicine Unit #5 employees will not perform maintenance or attempt repairs to equipment while such equipment is under the purview of this contract unless agreed to in writing by the Contractor. Maintenance Inspection Definition - One (1) scheduled inspection per year covering, but not limited to the Statement of Work outlined above by the qualified t echnician. Parts Availability - To ensure minimal downtime to equipment, the Contractor shall maintain replacement parts and materials necessary to perform each task or supply said parts and materials within 2 days. Compensation - Labor: 1) All compensation for labor is included in the contract price. 2) Parts & Material: All compensation for parts and materials is included in the contract price. 3) Parts / Supplies Quality: Parts and supplies provided under this contract shall be guaranteed to be equal in all respects, including performance, interchangeability, durability and quality to the OEM parts with new or as presently recommended by the manufacturer. This shall be a firm fixed price contract. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items applies. Addendum to FAR 52.212-1, Paragraph (b)(5): Offers shall provide an express warranty w hich at a minimum shall be the same warranty terms, including offers of extended warranties, offered to the general public. Express warranties shall be included in the contract. Clause 52.212-4, Contract Terms and Conditions - Commercial Items applies as well as the following addendum clauses: FAR 219-1, Small Business Program Representations; FAR 52.219-6, Notice of Total Small Business Set-Aside; FAR 52.211-5, Material Requirements; FAR 52.232-18, Availability of Funds; and FAR 52.232-19, Availability of Funds for the Next Fiscal Year. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, FAR 52.222-36, Affirmative Acti on for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, applies with the following clauses applicable for paragraph (b): DFARS 252.225-7001, Buy American Act and Balance of Payment Program; and DFARS 252.225-7012, Preference for Certain Domestic Commodities. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Certifications - Commercial Items. The government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factors. Parties responding to this solicitation may submit their quote in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include the following information: 1) Company's complete mailing and remittance addresses, discounts for prompt payment, if any (e.g. 1% 10 days), anticipated delivery / availability of product / s, the company's CAGE code, Dun & Bradstreet Number, and Taxpayer ID number. In addition, if you are quoting on a comparable commercial item, product literature must be included. All FAR certifications and representations specified above must also accompany your quote. Quotes must be received no later than 3:00 PM, local time on 11-14-02, and will be accepted via fax (619-532-2347) or via e-mail (jessica_e_dunker@sd.fisc.navy.mil). Quotes submitted, as an attachment to an e-mail should be sent in Word Version 6.0 or higher.
