Description
NA Capital District Contracting Center is issuing a combined synopsis solicitation for commercial items prepared in accordance with the format in FAR 12.6 supplemented with additional information included in this notice. Please request a written proposal via email to Denese_Y_Henson@belvoir.army.mil or 703-806-4446. This procurement is restricted to 100% small business set-aside. Solicitation number DABJ03-03-T-0011 and is issued as a Request for Quotation (RFQ). This solicitation document, incorporated pro visions, and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS code is 541618, size standard is 5.0 mil. Financial Readiness Trainer Counselor in accordance with statement of work, for Soldier and Family Support Center, Fo rt Belvoir, Virginia. Period of performance base year 18 November, 2002 thru 17 November 2003 for 800 hours.And one option year, also 800 hours. SPECIFIC REQUIREMENTS: STATEMENT OF WORK;C.1. GENERAL The purpose of the Financial Readiness Program (hereaf ter ?FRP?) is to establish and operate a service for soldiers and family members that focuses on improving the soldier?s personal financial status and their abilities to perform as informed consumers. C.2. SCOPE OF WORK. The FRP Contractor (hereafter referred to as ?contractor?) works independently within the Army Community Service (ACS), FRP. The contractor shall accomplish the following objectives:C.2.1. Contractor shall develop training that provi des financial education, information, and prevention services.C.2.1.1. Mandatory topics that shall be included in training for service members per the prevention and education required by AR 608-1 are as follows: Banking and Credit Union services,Budget De velopment and Record,Keeping Debt Liquidation,Credit,Consumer Rights and Obligations,Insurance,Personal Financial Management Readiness to include check management and PCS moves. C.2.1.2. Each training class shall have a class outline, lesson plan, class ma terial, and handouts. These materials shall be provided to the FRP Manager per DA ACS accreditation requirements. Upon completion of each class, the Contractor shall provide sign-in lists of attendees and their evaluation sheets to the FRP Manager.C.2.2. Individual counseling shall be available. C.2.2.1. Counseling services and emergency assistance shall be provided to service members, family members, retirees, and DOD civilians. Attendees/clients may either be self-referred or mandated by their commanders, walk-in, or phone callers. All clients must fill out the intake form prior to an individual appointment. Contractor shall use DA ACS required forms during all counseling sessions.C.3. OPERATING HOURS:C.3.1. Unit training w ill be scheduled on a request basis. Marketing directed at the unit commanders will help in scheduling.C.3.1.2. Unit training and community classes shall be coordinated through the FRP Manager.C.3.2. Individual counseling shall be conducted during regula r office hours, Monday ?Friday, 0745-1630.C.3.2.1. Counseling schedules shall be provided to the FRP Manager on a weekly basis.C.3.3. The Contractor shall not be paid for federal holidays.C.3.4. The Contractor shall submit a statement of services and lo g of daily hours worked MONTHLY to the RRP Manager. This report is due in conjunction with submission of Contractor?s invoice.C.4. QUALIFICATIONS. The contractor shall have the following:C.4.1. A Bachelor?s degree in Financial Planning Service or Finan cial Planning accreditation from the Department of the Army.C.4.2. More than five years experience as a Financial Counselor with Army Community Service, Financial Readiness Program, and/or Army Emergency Relief (AER). C.4.3. Knowledge and experience of the military system and lifestyle, theory, practices and problems involved in community organization, military and community resources, Army Community Service Centers, and related services or activities.C.4.4. Demonstra ted skill in com municating effectively with professionals, military and civilian personnel, family members, and groups.C.4.5. Demonstrated ability to identify and work with family problems and respond to any specific needs.C.5. GOVERNMENT SUPPLY ITEMS AND PROPERTY. C.5.1. The Government property, facilities, equipment and resource materials, made available under this contract shall be used only in performance of this contract.C.5.2. The Government will provide the contractor the following property for use in connection wit h this contract: One classroom to provide classes and to be shared with other staff. Copying and printing as needed of material to be covered in the classes and forms reviewed by the FRP Manager and approved by the Army Community Service Officer.Any neces sary equipment that is available, to include training aids, which supports the services to be delivered.Sufficient office space for one contractor to use on a part time basis. If necessary, the office will be shared but not at the same time. The Contract or shall have additional space for training, as needed, if space allows. Office furniture, to include a desk, chair, computer, and a filing cabinet. C.5.3. The Government will not be responsible in any way for damage to the contractor?s supplies, material s, or personal belongings brought into the building due to fire, theft, accident, or other disaster.C.5.4. Contractor shall be responsible for receiving, caring and accounting for any government furnished property. Shall be for use only in performance of this contract.C.6. RECORDKEEPING. Case files shall be maintained in accordance with Army Regulations AR25-400-2 and AR34-21 and stored in the FRP Manager?s office.C.7. CONFIDENTIALITY. Strict confidentiality shall be maintained regarding clients? record s and their personal information. Information may ONLY be released to other military agencies request of the client.C.8. VEHICLE REGISTRATION. Contractor shall obtain vehicle passes/decals at the Visitor Center prior to commencement of contract work.C.9. SAFETY. Contractor shall be aware of local safety policies and regulations to ensure that a ll reasonable efforts are made to protect all persons from harm and U.S. Government property from damage. C.9. TERMINATION PROCESS. Contract will be terminated if contractor does the following:C.9.1. Contractor fails to fulfill the contract as stated in the Statement of Work. C.9.1.2. Contractor is determined by the Government to be a security risk, under the influence of alcohol or drugs or is physically or mentally impaired to the extent that they cannot perform the stated tasks.C.9.1.3. Contractor breaks the rules of confi dentiality.C.9.2. Such determination is to be made within the sole discretion of the Contracting Officer or his/his designee. Such determination shall not relieve contractor of meeting the performance requirements of this contract.C.10. CONFLICT OF INTE REST DISCLOSURE STATEMENT. Enclosed must be signed by contractor.CONFLICT OF INTEREST DISCLOSURE STATEMENTAUTHORITY: DoD Directive 5500-7 (Joint of Ethics Regulation); Executive Order 9397 allows the use of the social security number as an individual iden tifier.PRINCIPAL PURPOSE and ROUTINE USE: This number is used as an additional means of identification among Department of Defense (DOD) employees. DISCLOSURE: Furnishing the social security number on this form is voluntary; however, failure to do so may result in personnel processing delays.SUBJECT: Disclosure of Actual or Apparent Conflicts of Interest by Personnel Performing Personal Financial Ma nagement Counseling to DoD Personnel and their Families.You are required to comply with the requirements of DOD Directive 55007.R, which establishes standards of conduct and ethical principles for all DOD employees.
