Description
12. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. All responses to this Solicitation/Request for Quotation (RFQ) must refer to Solicitation Number F04693-03-T-0003. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. This contract is Set-aside for Small Business. The North American Industry Code (NAIC) associated with this requirement is 713940, and the size standard is $6,000,000. Aerobics instruction will be held in two locations approximately 20 miles apart. The contractor will quote an hourly rate, and total quote for 1098 hours per year based on the following two (2) contract line items (CLIN). CLIN 0001?Base period of one year aerobics instruction, 22 hours per week. CLIN 0002?An optional period of one (1) additional year of aerobics instruction, 22 hours per week. The contractor shall provide all functions, tasks and responsibilities normally inherent in providing aerobics instruction in accordance with the attached statement of work. The attached Department of Labor Wage Determination No.: 1994-2047, Revision No.: 22, 09/10/2002 applies. Service locations are fitness centers at (1) the Los Angeles Air Force Base in El Segundo, California 90245, and (2) Fort MacArthur in San Pedro, California 90731. The anticipated start date is 01 December 2002 for a base period not to exceed one (1) year, and one (1) option period not to exceed one (1) year. Commercial Items?, including the following sub clauses, 52.222-3, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-41, 52.222-42, 52.222-43, 52.222-47, and 52.232-33, apply to this acquisition. All offerors are required to include a completed copy of FAR 52.212-3, ?Offeror Representations and Certifications-Commercial Items? with this offer. The full text of these clauses may be accessed electronically at http://farsite.hill.af.mil. The offeror shall submit copies of instructor certification with the offer. The Government will award a contract resulting from this solicitation to the responsible applicant whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Factor 1: Past Performance. Factor 2: Price. The offerors shall provide references from at least three (3) customers who use the contractor?s aerobics services. The contractor is required to have a minimum of one (1) year ?s experience in providing aerobics instruction. Provide relevant references from work performed within the last three years immediately upon decision to submit proposal. Include a point of contact (POC), contract number, and telephone number. When combined, all evaluation factors other than price are significantly more important than price. If, after evaluation of past performance information, two or more proposals are determined to be relatively equal, price will be the determining factor of award. All information must be submitted to the contact listed below by close of business November 18, 2002. All responsible sources may submit a quote. All quotes will be considered.
