- AMENDMENT 1 TO SOLICITATION 600-040-03 ACCOMPLISHES THE FOLLOWING: 1. Changes the announcement type from "Sole Source" to "Combined Synopsis/Solicitation"; 2. Establishes solicitation objective as NEGOTIATED, FIXED PRICE COMMERCIAL ITEM CONTRACT, for Base Year and Four (4) Option Years; 3. Provides detailed requirements and selected forms to facilitate preparation of offers (see attached form); and 4. Extends the closing time/date of offers to 12:00 noon, 11/20/2002. THIS IS A COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with subparts in the Federal Acquisition regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Based upon this Solicitation No. RFP 600-040-03, VA VISN 22 Network Business Center intends to negotiate a commercial item (FAR Part 12) contract for voice telecommunications service for Veterans Affairs Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357 SPECIFICATIONS. a. Circuit type: ISDN PRI T-1 (two-way), minimum quantity 5 b. Blocks of 100 DID #s, local to VAMC calling zone, MUST BE 3000 EXTENSION SERIES (i.e., 909/xxx-3xxx), minimum quantity 10, OR port existing numbers identified in subparagraph f of this section. c. Monthly service fee d. Expanded Exchange Service (same as Extended Area Service) package (to afford direct connectivity between Medical Center and associated/remotely located VA Community Based Outpatient Centers e. Due to critical nature of communications for medical care delivery, offerors must be either Local Exchange Carrier (LEC) or Competitive Local Exchange Carrier (CLEC) for user area. f. Cost to port over existing numbers in interim until requested permanent service can be installed. (Numbers to be ported: 909/825-7084, 909/422-3000 through 3299, and 909/777-3800 through 3899) g. Caller ID on incoming calls to local PBX h. Caller ID (name "Jerry L. Pettis VA" & main facility number) on outbound calls i. Capability to roll to number on another PRI from a different vendor if circuits are down j. No down time for installation/cut-over k. List of new DIDs, plus documentation of switch configuration requirements and geographic circuit routes to connect to vendor network, a minimum of 2 weeks prior to cut-over l. Description of no-charge assistance to VA facility in planning/publishing phone number changes and terms of warranty on work performed m. Trouble call on-site response time not to exceed 2 hours from time VA designated representative places call (at no additional charge). Note: This is neither a request for, nor are offers desired for long distance service.
