Description
This is an emergency combined synopsis/solicitation. Request for Quotation #RFQ 618-54-03 entitled =93Temporary Computed Radiography (CR) Technologist=94 (2 FTE=92s) is hereby issued for commercial items prepared in accordance with the format in FAR Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This procurement is being conducted under FAR Subpart 13.5 test program; simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-09 and VAAR 97-9. NAICS Code 561320 applies for this procurement and the small business size standard is $11.5 million. This BOS announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor=92s proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor=92s letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor=92s price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. The VA Medical Center (VAMC), Minneapolis, Minnesota, has identified an urgent requirement for two Computed Radiography (CR) Technologists for an estimated three to six month period scheduled shift time is Monday through Friday 8:00 a.m. to 4:30 p.m. with Holidays as assigned (Christmas a definite). The following is a list of (A) Duties and responsibilities: (1) Corroborates patient=92s clinical history with procedures ordered. Assures information is documented and available for use by a physician. (2) Respects confidentiality and follows established policy regarding patient records. (3) Assumes responsibility for specific job requirements and reports any equipment malfunctions. (4) May be responsible for stocking rooms with supplies and inventory in assigned areas. (5) Practices aseptic techniques as necessary. (6) Initiates basic life support action when necessary. (7) Assists in the day to day operation of the department in routine housekeeping and equipment care that the department is responsible for. (8) Is a registered technologist (ARRT) and maintains annual registry renewal. (9) Also pursues required continuing education as set by the registry. (B) Knowledge required by the position: (1) Knowledge of patient care practices such as =96 (i) Attitude and communication with veterans of various ages, dealing with their physical and mental limitation, safe lifting and transfer procedures, evaluating their physical needs, and infection control practices. (ii) Ability to communicate with physicians/surgeons and nurses in workflow coordination. (iii) Dealing with medical emergencies such as cardiac arrest, seizures, aspirations, hemorrhage, etc. (iv) Dealing with acute situations such as patients with head injuries, patients with spinal injury, anaphylactic reaction to contrast media, patients with barium studies, caring for patients with tube management such as oxygen, gastric tube, tissue drains, etc. (v) Has a thorough knowledge of and can apply principles of universal substance precautions. (2) Thorough knowledge of anatomy and physiology. Such as CR exam protocols for given history/0diagnosis/pathology. (3) CR procedures: Such as positioning for routine radiographic procedures; positioning for routine contrast studies; and positioning for routine surgical procedures and general orthopedic examinations. Thorough knowledge of the principles of Computed Radiography (CR)(to include the exposure and processing algorithms and how pathologies are best demonstrated) (4) Minimum of Three years experience as a CR Technologist, Ability to teach and mentor students (5) Dedicated experience in a Digital environment. (6) Lead Technologist or Management experiences a plus. (7) Imaging equipment; Fugi computed Radiography (CR) system, Picture Archiving Communications (PACS) workstation (to include the ability to do basic trouble shooting of image/network flow,) C-Arms, Portable X-ray, Fluoroscopy (8) Radiation protection; personnel monitoring and occupational exposures; patient protection. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Contractor's employees must be present at VAMC and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle.=20 A VAMC record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. The payments to contract personnel for any leave, including sick leave, holiday, or vacation time, is the responsibility of the contractor.=20 Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during technologist=92s absence. Evaluation of proposals offered will include the following factors: Professional qualifications, clinical skills/interpersonal skills, and availability, past performance history (include three corporate or Government references with phone numbers); and price. Award will be made to the offeror determined to be the =93best value=94 to the Government. Awarded offeror is hereby notified that the position would be considered =93low risk=94 under the positions sensitivity designate IAW IL 90-01-6 entitled Contractor Personnel Security Requirements. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
