Description
This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR subpart 12.6 Streamlined Procedures for evaluation and solicitation for commercial items as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-09. The associated North American Industrial Classification System (NAICS) code for this procurement is 524126. This acquisition is restricted to small business only. The Food and Drug Administration (FDA) has a requirement for Travelers Insurance for 550 FDA traveling inspectors. The coverage shall include: emergency evacuation, medically necessary repatriation, repatriation of remains, medical escort, visit of a family member or friend, return of recreational vehicle/auto and accidental death and dismemberment. Offerors shall include expense limits and usage limits with literature which CLEARLY DOCUMENTS that the offered product meets or exceeds the specifications stated. The Period of performance is for one year. The following provision & clauses apply to this acquisition: 52.212-1, Instructions to Offerors-Commercial Items; and 52.212-3 Offeror Representations and Certifications-Commercial Items. The following clauses apply to this acquisition: 52.212-4, Contract Terms and Conditions-Commercial Items. All offerors must submit the following: 1) Two (2) copies of a quotation; 2) Technical description and/or product literature; 3) A copy of the most recent published price list(s); and 5) a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items which may be downloaded at www.arnet.gove/far. All sources must submit along with their written quotes a nine digit numeric DUNS number that is assigned by Dun & Bradstreet. To apply for DUNS number please call 1-800-333-0505. Fax and e-mail quotes will be accepted. All quotes shall be sent to The Food and Drug Administration, Office of Acquisitions Facilities and Central Services, Attn: Dee Gibson, Room 2131, 5600 Fishers Lane, Rockville, MD 20857. Submission must be received by 5:00p.m. local time on November 22, 2002. Fax and e-mail quotes will be accepted.
