- Dover Air Force Base wishes to establish a Reoccurring Service Order to have a contractor provide a Protestant Accompanist for the 1700 Saturday and the 1100 Sunday services, complete specifications and Statement of Work are available as attachments to this solicitation. The Contractor Performance Survey must be completed by contractor?s previous employer and faxed or mailed with contractors bid. This procurement is restricted to 100% small business concerns and will follow the simplified acquisition procedures in FAR Part 13. This solicitation is a Request for Quote (RFQ). This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Submit written quotes; oral offers will not be accepted. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. FOB: Destination for delivery is Dover AFB DE 19902-5639 Base Chapel building 419 and housing chapel building 2998. Note that all FAR clauses can be obtained by accessing the Internet at http://farsite.hill.af.mil. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, apply to this solicitation. The clause at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Terms, applies to this solicitation. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-27, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-42, Statement of Equivalent Rates for Federal Hires 52.222-48. The clause at DFAR 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this solicitation. Specifically, the following clauses are cited to this solicitation: DFARS 252.225-7001, Buy American Act and Balance Payment Program. All interested parties must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, and must be registered with the Central Contractor Registration (CCR) as prescribed in DFARS 252.204-7004 & 252.232-7009. CCR can be obtained by accessing the Internet at hhtp://www.ccr.gov or by calling 1-888-227-2423. The CCR number must be obtained before award can be made. Award will be based on lowest price and best value determination. The Government reserves the right to make an award on the initial quote without discussions on this procurement. All proposals must be faxed to 302-677-2309 or e-mailed to marland.clark@dover.af.mil , 639 Atlantic Ave, Dover AFB DE 19902-5639. The due date for all quotes will be 06 DEC 2002 at 4:00 p.m. EST. Quotes must be good for 90 days. The terms and Conditions for this Purchase Order will be set-up for a one-year term.
