- Description
- Request for Information (RFI) ? Family Advocacy Program (East CONUS) The Air Force Medical Operations Agency (AFMOA) at Brooks AFB is in the process of determining the acquisition strategy for the Family Advocacy Program for the East CONUS. The team is soliciting your comments to the questions listed below. We would greatly appreciate any feedback industry has to offer. The draft Statement of Work (SOW) is attached. This RFI is not a formal solicitation and is not a request for proposals (RFP). Any formal solicitation will be announced separately. This RFI is for information and planning purposes only and is not to be construed as a commitment by the Government. We intend to follow this RFI with the release of a draft RFP for industry comment. The Air Force Family Advocacy Program (FAP) provides a continuum of services that include prevention and treatment for child and spouse maltreatment with military families. The contractor shall provide qualified clinical master-level social workers, U.S. license registered nurses, and family advocacy program staff personnel to perform services of the nature and scope described in the draft SOW. - Provides intervention and prevention of spouse and child maltreatment in military families at select Air Force bases (AFBs) in the East CONUS region (AFBs are listed in the draft SOW). - Personal services - NAICS code: 624190 - Anticipate a 1 year basic period with four one-year options - Firm Fixed Price, Indefinite quantity - Rough Order of Magnitude of approximately $13M for total period of performance - Tentative CLIN structure: - CLIN 0001: Family Advocacy Treatment Manager (FATM) - CLIN 0002: Family Advocacy Intervention Specialist (FAIS) - CLIN 0003: Family Advocacy Outreach Manager (FAOM) - CLIN 0004: Family Advocacy Nurse (FAN) - CLIN 0005: Family Advocacy Program Assistant (FAPA) - CLIN 0006: Travel Costs for Training ? travel, per diem and registration fee, when applicable; per diem rates as those authorized in the Joint Travel Regulation (JTR) - CLIN 0007: Travel Costs for Vicinity travel ? home visits to families in accordance with draft SOW; rates as authorized in the JTR - Start date is anticipated for 01 October 2003 Please provide answers to the following questions. Your responses will be used in market research for this acquisition.
