This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued; other than in an electronic format. This Performance-Base Service Con tract is for non-personal services. The Performance of Work description, Performance and Evaluation Criteria and the Quality Assurance Plan is attached for the Contractor to determine how to best meet the government's performance objectives. CLIN 0001: The requirement is for the following period, Jan 2002 and ending 31 Jul 2003. CLIN 0002: Two Option Months: Option 1: 01 Aug 03 to 31 Aug 03; Option 2: 01 Sept 03 to 30 Sept 03; CLIN 0003: 1 Option Year: CLIN 0004: 6 Month Option Period Extension: CLIN 0005: 3 Month Option Period Continuity of Services. Option periods will be based on the availability of funds. FAR 32.703-2(b) Note: Each CLIN and period is required to be quoted separately. Then consolidate the totals into a one single price on page one, Block 26, on the SF Form 1449 Solicitation. The Family Support and Readiness programs are essential to the life support of Airmen and their families on behalf of the Delaware Air National Guard and in coordination with the State Family Program Coordinator to th e Delaware National Guard Family Programs Office. The Family Support and Readiness Program Contractor provides counseling before, during and after the deployment, specialized training for pre/post deployments; provide further development, writing or assist ance in improving the Mobilization Assistance Plan, MAP. In addition, this office would have the ability to assist active duty remote recruiters/active duty pilots and other personnel located at or near Air/Army National Guard Units and their families and provide family support to active duty retirees within close proximity to Air National Guard communities. The MAP guide book of the 166 Airlift Wing provides the guidelines of the benefits and services at the Delaware Air National Guard; Directory of Air Na tional Guard Services, Directory of Community Resources and Support agencies, Community Resources and Support Agencies, Family Care Planning, Tips on Preparing the Family for Separation, Financial Matters, Legal Matters, Insurance, Preventative Maintenance Planning, and other miscellaneous or important information with regard to special instruction for Spouse or Caregivers too include important document, pamphlets or handouts. The MAP guidebook can be reviewed at the Delaware Air National Guard Contracting Office. The MAP will not be distributed. Requirements for this contract are in accordance with the following applicable publications and forms and are included as attachments: ANGR 211-1/NGR 600-12, National Guard Family Program; AFI 36-3009, Family Suppor t Center Program; ACC or AMC Supplement or MAJCOM Policy Letters to AFI 36-3009; AFI 90-500, Community Action Information Board (CAIB) and Integrated Delivery System. The contractor may be asked to perform other services as indicated in section C-5 in acco rdance with the MAP. Contractor shall provide services during the general operating hours of the 166th Air Wing/ Delaware Air National Guard, 2600 Spruance Drive, New Castle, DE 19720-1615. Monday through Friday, 0700-1630 Note: See UTA/ Scheduled Days Off and Holiday Schedule. The established performance of work months for this contract is normally the first day of the month through the last day of each month. Source selection procedures in accor dance with FAR 15.3 shall be utilized. Significant criteria in descending order of importance are: 1 Technical; 2 Past Performance; 3 Price as outlines in section C-16. Oral Presentations are required to be presented at a designated date and time. Time l ength of the presentation is limited to 20 Minutes. The Source Selection Team will review the Contractor's Pre-submitted proposal prior to the presentation.
