NA THIS IS A SOURCES SOUGHT NOTICE. FOR FURTHER INFORMAITON CONTACT MS SHELIA KAYLOR, RN, NURSE METHODS ANALYST AT 706-787-2019, EMAIL: Shelia.Kaylor@se.amedd.army.mil This Federal Business Opportunity announcement is a sources sought notice that TRICARE Regions 3 & 4 which includes all DoD Medical Treatment Facilities (MTFs) in the states of Alabama, Georgia, Florida, Mississippi, South Carolina, and Tennessee are in t he process of standardizing monitoring electrodes as listed in the subject line above. These include: adult and pediatric continuous monitoring electrodes, radiolucent electrodes, and EKG electrodes. Companies with current Distribution and Pricing Agreem ents (DAPA?s) with the Defense Supply Center Philadelphia for these items are eligible to participate in the standardization process. Base company criteria for this product line includes but is not limited to the following: must carry a full product line , must provide customer support in the form of a sales representative for each MTF within TRICARE Regions 3 & 4, must provide clinical support, education programs, and utilization reports, must provide company?s return policy, must provide company?s distri bution network, must be able to provide product for all MTFs within TRICARE Regions 3 & 4, and must provide electronic and hard-copy of proposed pricing and competitive product cross-reference. Base product criteria for this product line includes but is n ot limited to the following: must provide excellent trace quality, secure adhesion, latex-free, hypoallergenic / no adverse skin reaction, comfortable for patients, adult & pediatric sizes, radiolucent products available, repositionable, fast - efficient c onnection, packaged in strips of 3, 5, or bulk quantity, secure lead attachment with monitoring equipment, and provide 12-Lead EKG tabs. Respondents must address all criteria listed above. Please send your response along with product literature and repre sentative product samples (enough to share with a 10 person Clinical Evaluation Team) of your company?s Monitoring electrodes and EKG electrodes to Ms. Sheila Kaylor, RN at the following address: Southeast Regional Medical Command BLDG 40707, 40th Street Ft.
