- (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation of Purchase Request F73HOS23250200 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-05. (iv) This acquisition is set-a-side for small business. The associated NAICS code is 621512 and small business size standard is $11,5000,000.00 (v & vi) This is a BRAND NAME request. The service requested is for Acceptance testing for the 2 Phillips ADAC Nuclear Medicine imaging systems to insure that the systems will meet radiation and diagnostic interpretation standards. (vii) FAR clause 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (viii) Offerors are advised to include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items, with the quote. Offerors must also include a completed copy of the provision at DFARS 252.212-7000, Offeror Representation. (ix) FAR clause 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition. (x) FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-- Commercial Items, applies to this acquisition.
