- Description
- Provide Architect/Engineer professional engineering services for Desi= gn Development and Construction Documents =96 complete drawings, specifications, cost estimates, construction period services, and construction site visits to renovate and modernize the Emergency Department Project No. 516-320 at the Department of Veterans Affairs VA Medical Center 10,000 Bay Pines Blvd. Bay Pines, FL 33744. Services to Include: Development of program space requirements, study of existing equipment and furniture, conceptual design layout schemes, Design Development Drawings, Working Drawings, Specifications, Cost Estimate and Construction Period Services to renovate approximately 16,500 square feet of existing space on the first floor of the main hospital building 100. The renovation is to include but not limited to modernizing and expanding the emergency department by increasing the size of the waiting area, to create a four (4) bed Chest Pain Evaluation Unit, expand treatment beds from eight (8) to thirteen (13) and create an additional Psychiatric Observation Unit (4 beds). To acquire this additional space on the first floor of building 100 the existing Orthopedic Clinic will move to the third floor Surgery area. Also, the existing Outpatient Radiology will be relocated to second floor to allow for the required space for this project. Space has been identified for these domino moves. The modernization may include new flooring, walls, ceilings, mechanical, electrical and communications systems. The estimated construction cost is between $2,000.000 to $3,000,000. Project construction completion is scheduled for FY 04/05.=20 NOTE: This is a request for SF 254 and SF 255 only. Any requests for solicitation will not receive a response. To be considered for selection, interested, eligible firms shall submit three (3) copies of their SF 254 and 255 no later than January 29, 2003 by 3:00PM local time and shall be addressed to: VA MEDICAL CENTER PO BOX 457, ATTN: CONSTANCE SPRINGS (90G) BAY PINES, FL 33744. Eligible A/E firms must be located within 50-mile radius of Bay Pines, FL. This is a small business Set aside. The North American Industry Classification System (NAICS) Codes are 541310/541330. The Small Business size standard is $4,000,000.00 in annual gross revenues averaged over the past three years. The A/E selection criteria shall include the following elements (a) through (k) (not necessarily listed in order of importance or merit): a) Team proposed for the contract b) Proposed management Plan; c) Previous Experience; d) Facilities location; e) Cost Control Techniques; f) Estimating Effectiveness; g) Schedule control; h) Describe your experience and capabilities in the following areas: Value engineering; Life Cycle Analysis; Critical path method; Fast-track-construction; Energy conservation; New energy resources; Environmental assessment; Specialized experience; Computer aided design, drafting, and other computer applications; i) Describe awards received j) Describe circumstances and outcomes of any litigation you have been involved in over the last five (5) years; k) Indicate type and amount of liability insurance you carry. A/E firms having a current SF 254 and 255 on file with the VA are still required to submit these forms. THIS IS NOT A REQUEST FOR PROPOSAL.
