Description

This Sources Sought/Market Survey Notice is being conducted by 460th Contracting Squadron, Buckley Air Force Base, Colorado to determine the size, type and billing methods of businesses able to provide Employee Assistance Program (EAP) Services to Buckley Federal Employees and their family members, to the extent feasible. The EAPs is for employees who have alcohol and/or drug problems that are seeking rehabilitation and the opportunity to become fully productive members of the workforce. In addition to substance abuse problems, EAPs provide comprehensive counseling and referral services to help employees achieve a balance between their work and family and other personal responsibilities. (http://www.opm.gov/ehs/eappage.asp) These services need to be provided to approximately 350 Federal Employees on an as required basis at an off-site location beginning approximately 17 January - 30 September 2003, plus possible option years. If this is a service your business can provide respond to this Sources Sought by 10 January 2003. Responses should include: (1) Business Name (2) Point of Contact Name and Phone Number (3) DUNS and NAICS code(s) (4) Business Size SMALL or LARGE (5) Type of Business 8A, HUBZone, WOMEN-OWNED etc. (6) Billing Methods LUMP SUM, HOURLY etc. (7) Estimated price for service.
