Description
NA This is a pre-solicitation notice. Contractor must provide professional non-personal medical services, lab/chemical services, equipment and supplies required to provide periodic medical surveillance examinations and reports at seven locations within the S tate of Michigan for the Army National Guard. The locations are: Ishpeming, Grayling, Bay City, Taylor, Grand Ledge, Lansing, and Augusta. All services will be performed in a mobile lab owned and operated by the contractor. The following services are r equired at each location: physical exams (complete occupational health history, physical exam, dermatological exam, anthropometric measurements, vital signs, vision screen), complete blood count with differentials, blood chemistries (22 item panel that in cludes HDL and LDL), PSA antigen, on board urinalysis, audiometry and respirator usage clearance, pulmonary function testing, blood lead with zinc protoporphrin, one-view (PA) chest X-ray with B-reader, and audiometric testing. All procedures will be cond ucted in accordance with the American Board of Occupational and Environmental Medicine, American Health Administration and the National Institute for Occupational Safety and Health Standards. A board certified occupational medicine physician and certified occupational health nurse shall be available at each site. Current estimated employees requiring these services are 437. Medical exam reports showing test results of each employee will be required from the contractor in disk format (Occupational Health Manager (OHM) software capable). The contract will be an Blanket Purchase Agreement contract for a base period commencing March 1, 2003 to September 30, 2003 and will include two one-year option periods renewable at the beginning of the federal fiscal yea r (October 1). North American Industry Classification System code is 621999. Standard Industry Code is 8099. The small business size standard is $8.5 million. The solicitation will be available on or about January 31, 2003. The following factors will be used to evaluate offers: (1) Past Performance ? experience with similar type contracts, OHM experience/compatibility, (2) Technical ? mobile van configuration, certifications, (3) Price.
