- Description
- This notice is part of the market survey to locate potential sources for Contractor(s) to provide Community Based Primary Care clinical space in the city of New Castle, Lawrence County, Pennsylvania to the Department of Veterans Affairs Medical Center, Butler, Pennsylvania, for the period of one year from the date of award in accordance with the requirements of this solicitation. Contractor=92s space must be physically located in the city of New Castle, Lawrence County, PA.=20 Space to include, but not limited to three (3) examination rooms, approximately 100 square feet each, with operating sink, hot and cold running water with foot operated controls, adequate lighting, two electrical outlets and one telephone outlet. Three (3) modem lines are required. At least two (2) examination rooms are to have modem lines for laptops; one may be located in an administrative area of the clinical space. The use of a waiting room for veteran patients, easily accessible from patient parking areas. The use of a patient registration area, administrative space for records and files, telephone outlets for facsimile and copier machines and computer terminals. The use of a lighted staff room, approximately 100 square feet, with two electrical outlets and one telephone outlet. A minimum of one restroom proximate to or included for use by patients and staff, approximately 65 square feet. Restroom shall include a sink, water closet, urinal, mirror, grab bars, soap dispenser and paper dispenser.=20 All space must be accessible to the handicapped in accordance with the Uniform Federal Accessibility Standards (41 CFR 101-19.6, App.A). This contract will include three (3) one-year renewal options that may be exercised at the option of the Government at the same terms and conditions. The VA anticipates utilizing clinical space at least one (1) day per week with the option to increase the use of space up to four (4) days per week. Frequency of use will be determined based upon response to this solicitation. The VA anticipates each clinic day to be between 6-8 hours; time approximately 9:00 AM =96 3:00 PM. Interested sources must contact the Contracting Officer no later than January 21, 2003.
