Description
NA This is a combined synopsis and no other solicitation will be issued. Services to be provided per the below Performance Work Statement: ARMY COMMUNITY SERVICE FAMILY ADVOCACY PROGRAM C.1 GENERAL Army Community Service (ACS) programs are essential to the life support of soldiers and their families. ACS Family Advocacy Program is responsible for the overall well-being of the military families regarding family violence prevention. Specialized traini ng for unit commanders, NCOs, military units, personnel working with children and families, such as Child and Youth Service Programs, Early Development Intervention Services, Antilles Consolidated School System, and any agency working with children and/or families. Parenting support groups and classes/workshops regarding effective parenting skills are also provided. Provide assistance with shelters, respite care, education, training, locating areas on and off post, resource for educational information and administration information and clerical assistance. The education program assists active duty, reserve service members and DoD civilians and their families to receive appropriate training and information regarding parenting skills and family violence prevention. Requirements for this contract are in accordance with AR 608-1, Army Community Service Center, which mandates all Family Advocacy Program to offer mandated training to military personnel and their families as well as those working with children and familie s. C-2 CONTRACT SERVICES C.2.1 The contractor shall perform specific services as indicated in Section C 5. Contractor will provide services 30 hours per week, generally during the operating hours of the ACS office (Monday through Friday, 0700-1600, from 15 January 2003 through 15 January 2004). Contract may be renewed (2nd option year, 15 January 2004 through 15 January 2005) at the option of the government. C.2.2 Contractor services are not required on legal public holidays (see C 7), during closure of the post and/or the facility for emergency situations, and officially declared military training. Contractor will be expected to alter work schedule if necessa ry during weeks that include a holiday. C.2.3 Contractor shall make no private use of any government facilities or other government property in connection with this contract. C.2.4 Contractor shall submit to an Installation Records Check (IRC) in accordance with regulation. The IRC includes a screening of past law enforcement and other records, e.g., checks by Criminal Investigation Division, Army Central Registry and Army Alco hol and Substance Abuse Program. Contractor will sign an authorization form to initiate this process and contract will be contingent upon satisfactory findings as determined by the Contracting Office Representative and Family Advocacy Program Manager. C.2.5 Services must adhere to standards of practice set forth by the U .S. Army and relevant federal, state, and local law. Contractor must demonstrate sound professional judgment and highest ethical standards in executing contract responsibilities. C.2.6 The Army Community Service Director will be the Contracting Officer's Representative(COR) responsible for review of contract requirements. FAPM will assign/review work and monitor ongoing work with families and groups. FAPM will review work on a re gular and ongoing basis. C.2.7 Contractor will maintain a neat, well-groomed appearance and workspace at all times. C-3 CREDENTIALS C.3.1 Specialized Work Experience: a. Minimum (1) year experience working with families and children, basic knowledge of office equipment operations. b. Work experience could be military, civilian, and/or volunteer. C.3.2 General Experience: Minimum of one (1) year experience in military lifestyle (e.g., prior work experience within a military community service agency, military families or military service. C.3.3 Education: Minimum: Bachelor Degree in Counse ling, Social Work or/and Psychology. Maximum: Master Degree in Counseling, Social Work or/and Psychology. Combination of experience and education will be considered. C.3.4. Related Knowledge, Skills, and Abilities: a. Knowledge of basic office skills (typing, Microsoft and basic computer, inventory of training material). b. Working knowledge of local military and civilian resources to support families and ability to work cooperatively with referral agencies to connect military families with needed services. c. Familiarity with ACS Family Advocacy Program; ability to exercise sound judgment and perform professionally and ethically IAW regulation and local ECS policies and procedures. d. Demonstrated ability to develop trusting relationships and work effectively with individuals and families from diverse racial, ethnic, and socioeconomic backgrounds, sometimes in crisis situations. Contractor must be able to operate independently to as sess client and community needs and develop, execute, and evaluate plans to address those needs. e. Proficiency in oral and written communication skills (English and Spanish). f. Physical ability to work with mid to heavy supplies in a safe manner. g. Contractor will hold a current motor vehicle license and use his/her own vehicle for business conducted within Fort Buchanan premises. Be able to acquire a military vehicle license and operate military vehicles as needed. C-4 COMPENSATION C.4.1 For satisfactory completion of the services under this contract, the contractor must be paid monthly, at the end of the month, until expiration/termination of this contract. C.4.2 Contractor shall furnish the Contracting Officer's Representation (COR) his/her invoice at the end of each work month. Nonperformance hours for absences other than identified in C.2.2 above shall be shown on invoice. Dividing monthly rate of pay by 1 72 and multiplying by number of nonperformance hours will make Reduction(s) from monthly pay for each nonperformance hour. C-5 SPECIFIC REQUIREMENTS C.5.1 Maintain FAP library organized and equipped with teaching material. Ensure material is available for classes and training for participants at all times. Keeps library and work area clean and neat. Assist clients who wish to borrow/return items from/t o the library. Prepares after action reports of classes and submits to the FAPM weekly each Friday. As a minimum, an annual inventory of library items shall be submitted by the contractor to the FAPM NLT 30 days after the beginning of the contract. C.5.2 Contractor will maintain a support group for parents. Will coordinate mandated training with every military unit’s education coordinator and with ACSS and CYS personnel. Contractor will disseminate information and flyers of classes through the inst allation. C.5.3 Performs one or more of the following duties: receives visitors and phone calls, and refers them to the appropriate person(s), office; answers routine inquires; schedules appointments with the aid of an appointment calendar; maintains a follow-up sys tem to ensure timely completion; creates/maintains filing systems and files a variety of documents; orders/maintains office supplies and other materials; provides office supplies and materials when requested; updates/maintains any type of manual and or automated log and operates fax machines and copy machines. Prepares information binders for classes/workshops, including, agenda, sign-in sheet, evaluation, and others. Agenda, sign-in sheet, and evaluation would be filed and kept on the FAPM office following each class/workshop. C.5.4 Assist and accompany the FAPM to different activities as required. Activities are - April: Child Abuse Awareness Month, October: Domestic Violence Awareness Month, December: Adoption Awareness Month, special training/workshop, and any other activi ty as deemed necessary. Responsible for conducting training or activities alone in the absenc e of the FAPM or when requested to. C.5.5 Uses office automation equipment and software to a limited degree. Contractor performs the following duties using typical automation equipment: type labels, envelopes and various forms, print messages from electronic mail. Contractor uses software su ch as electronic mail to receive and transmit messages and uses database management software to establish and maintain databases. Also, operates electric typewriter, office facsimile machine and office copying machine, providing services as requested. C.5.6 Provides assistance to the FAP Manager. Assistance may include performing one or more of the following duties: gathering data for reports, studies, referring to dictionaries, manuals, and other guides and running errands inside/outside office unit. C-6 REMOVAL OF CONTRACT PERSONNEL C.6.1 The Government will have the right to cause the contractor to replace any individual, functioning as a supervisor or employee, who is determined by the Government to be a security risk, under the influence of alcohol or drugs, or is physically or men tally impaired to the extent that they cannot perform the tasks established by the SOW. Such determination to be made within the sole discretion of the Contracting Officer or his/her designee. Such determination will not relieve contractor of meeting the performance requirements of this contract. C-7 LEGAL HOLIDAYS C. 7.1 Contractor shall not perform on legal federal holidays. They are as follow: January 1- New Year's Day January 15 - Dr. Martin Luther King's Birthday Third Monday in February -President's Day Last Monday in May -Memorial Day July 4 - Independence Day First Monday in September -Labor Day Second Monday in October -Columbus Day November 11 Veteran's Day Fourth Thursday in November -Thanksgiving Day December 25 - Christmas Day C-8 GOVERNMENT FUNISHED PROPERTY AND SERVICES C.8.1 The Government will furnish office space in Building 1019, Fort Buchanan, P.R. This includes a desk, a chair, filing cabinet, access to a computer and copier, and all office supplies necessary to comply with this SOW. C.8.2 The Government will furnish all Army Regulations, forms, and resource files necessary to comply with this SOW. C.8.3 The Government will furnish fire protection, telephone, utilities, custodial and security services to the extent that they are available to ACS, Building 1019, Fort Buchanan, P. R. C.8.4 The Government will furnish postal and installation distribution services for all official items. C.8.5 The government property made available under this contract shall be for use only in the performance of the contract. C.8.6 The Government will not be responsible in any way for damage to the contractor’s supplies, materials, equipment, or the contractor’s employees’ personal belongings due to fire, theft, accident, or other disaster. C.8.7 Contractor will be eligible to participate in professional training deemed by COR to be in the best interest of the government as funding allows. Training must be approved by COR. C-9 CONTRACTOR FURNISHED ITEMS C.9.1 Contractor will furnish his/her own transportation. It will be necessary for the contractor to use his/her own vehic1e to coordinate tasks/actions with military and civilian/agencies and organizations within USARSO, Puerto Rico, theater of operations . Use of government vehicle for trips to Roosevelt Roads Naval Station and environs is at the discretion of COR and dependent on availability of vehicle. C.9.2 Contractor must provide and maintain a cellular phone for communication with the office while in transit or on authorized trips outside Ft Buchanan. Cellular phone is required for purposes of personal safety and professional accountability. C.9.3 Contractor is not an employee of the federal government. Contractor will be responsible for all income taxes required by law. --------------------------------------------- Awardee mus t be CCR registered. Registration can be obtained at the website: www.ccr.gov. Payment to be issued via EFT or credit card. Awardee shall be subject and pass a background check. Quotes will only be accepted via fax at 787 707-3957. No specific format i s required to provide a quote. However, quote must be itemized ie. per service, per hour, per day, per month.
