Description
NA Non-personal Occupational Medical and Audiology Services for employees of Yakima Training Center, Yakima, Washington, for a base period beginning on or about 1 February 2003 through 31 January 2003 plus four subsequent option years. Solicitation number DA DA13-03-T-0029 applies to this action and is expected to be released on or about 28 January 2003. Services include the following: 1), Administrative medical examinations, such as pre-placement, periodic and termination exams. The estimated number of emplo yees who will need evaluations is 130 per year. 2), Hearing conservation education, protector fittings and referrals to an Audiologist when required. The estimated number of audiology referrals is 14 per year., 3), Occupational vision examinations, 4), J ob-related immunizations, 5), Employee education concerning job-related health hazards, 6), Medical screening for respirator programs, and 7), Return to duty evaluations, and 8), Examples of services required include physical examinations, blood tests, c hest x-rays with B reading, venipuncture, pulmonary function tests, vision screenings, urinalyses, specialty referrals and other procedures as required by clinical judgment. At least one physician shall have Board Certification in Emergency Medicine or oth er Acute Care Specialty. Services may also be provided by other physicians, and physician assistants under the direction of the board certified physician. Qualifications are as follows: 1), Current unrestricted license to practice medicine in State of Wa shington, 2), The physician director will maintain board certification in Emergency Medicine or an Acute Care Specialty. 3), For physicians, graduation from a Liaison Committee on Medical Education (LCME) approved U.S. or Canadian medical school or gradu ated from a foreign medical school listed by the World Health Organization, and 4), Physician assistants shall be certified by the American Commission for Certification of Physician Assistants. All services shall be performed at the Contractor?s location. Solicitation is expected to be released on or about 28 January 2003. CCR registration is required.
