- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. Solicitation DABJ13-03-T-0030 is issued as a request for quote (RFQ). This solicitation is a small business set-aside. Description: Prosthodontist and general dental services at Fort Carson, CO. The contract shall be for a period of one year, from 01 Apr 2003 through 31 Mar 2004. Requirement: Possess a current dental license in the U.S. and possess a valid Drug Enforcement Agency number to write prescriptions for controlled substances. Other requi rements to be in accordance with a statement of work that's available upon request. The vendor shall include a complete provision of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items.
