- Description
- This is a combined synopsis/solicitation for commercial items or services prepared in accordance the Format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) 000519. The solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code for this acquisition is 6512 and the NAICS Code is 621511. The Food and Drug Administration intends to issue a purchase order on a sole source basis to AB BioDisk North America, Inc. This purchase order will be for the following test strips: (1) Azithromycin-5100-0168, (2) Ciprofloxacin-5100-0868, (3) Chloramphenicol-5100-0758, (4) Clindamycin-5100-0958, (5) Erythromycin-5100-1058, (6) Gentamicin low-5100-1258, (7) Naladixic Acid-5100-1658, (8) Tetracycline-5100-2258 to be used by the National Antimicrobial Resistance Monitoring System (NARMS). The product is to be delivered to 950 College Station Road, Athens, Georgia 30605 and 1600 Clifton Road, Atlanta, Georgia 30333 and any shipping charges must be included in the cost proposal. The following FAR Clauses shall apply to this solicitation: FAR Part 52.212-1 Instructions to Offerors-Commercial Items, 52.212-3 Offerors Representations and Certifications, 52.212-4 Contract Terms and Conditions. All responsible sources that can provide and meet the above requirement shall submit written quotations by mail or by fax (301) 827-7103. All questions shall be submitted in writing by fax or email: ghubbard @ oc.fda.gov. All sources must submit along with their written quotes a nine-digit numeric Duns number that is assigned by Dun & Bradstreet. To apply for a Duns # please call 1-800-333-0505. Evaluation/Award will be based on the technically acceptable quote that offers the best value to the government. The solicitation is issued on a sole source basis AB BioDisk North America, Inc. is the only known source that can provide these E-test strips. It is very important for the NARMS program that the test strips have consistency and continuity of methods for the laboratories over the entire period of time to establish trends.
