Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is F5HOSP23650100 and is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisitions Circular 01-12. (iv) Notice: This solicitation is issued for full and open competition. The set-aside for Small Businesses has been dissolved. The NAICS code 541611 with a size standard of $6.0 million dollars (v) Contract line items are numbered 0001 for Base Year 01 Jul 03 to 30 Jun 04, 0002 Option Year One from 01 Jul 04 to 30 Jun 05, 0003 Option Year Two from 01 Jul 05 to 30 Jun 06, 0004 Option Year Three from 01 Jul 06 to 30 Jun 07, 0005 Option Year Four 01 Jul 06 to 30 Jun 08 per Performance Work Statement for Auditor/Coder services. (vi) Description of requirements for the items to be acquired- See attached Performance Work Statement. (vii) Required performance shall be between 01 July 03 and 30 July 2004. Place of delivery shall be 5 MDSS/SGSAR, Bldg 194, 10 Missile Ave, Minot AFB, ND 58705-5024. FOB: Destination. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. (ix) The provision at 52.212-2, Evaluation -- Commercial Items is applicable to this procurement. The evaluation procedure to be used for this procurement is price and price related factors. 52.217-5 -- Evaluation of Options. As prescribed in 17.208(c)(1), insert a provision substantially the same as the following: Evaluation of Options (Jul 1990) Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s). (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with offer. (see attachments) (xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable to the acquisition: 52.222-19, 21,26,35,36,37,- 52.225-1,- 52.232-18, 33. (xiii) Any additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are included in the attached Performance Work Statement.
