NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. DAHA25-03-T-0002 is being issued as a Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquistion Circular 01 -12. This solicitation has been set-aside 100& for small businesses. The NAICS Code is 333319. Brand name or equal. Minimum Specifications are CLIN 0001: Quantity 1 embroidery machine to include: One (1) each Elite 15-needle embroidery machine with cap frames, 12cm, 15cm, and 300x430 hoops (2 each) with COMPUCON CREATOR ADVANCED digitizing software, Barudan p/n BENT-ZQ-201U; One (1) each cart for Elite, Barudan p/n P710950; Four (4) each wheels for Elite, Barudan p/n A100588; One (1) each Elite extra hoo p set including two 9cm, two 18cm, and two 290x300 hoops, Barudan p/n NB 1002; One (1) each supply kit including thread, backing, needles, scissors; One (1) each OTT-LITE true color lamp with 3X magnifier; One (1) each Wahl embroidery stitch eraser: Four ( 4) each sewing hook, Teflon Mark V (KHS12-RYPBNW), Barudan p/n K220710; Twelve (12) each standard bobbin case, Barudan p/n KF221020; Two (2) each extra QS right clips for 520NS, Barudan p/n FR605070; Two (2) extra QS left clips for 520NS, Barudan p/n FR605 080; One (1) each serial cable include with software, Barudan p/n ZN30140; Sixty (60) each Marathon thread (misc colors); On-site installation/machine training. Deliver to Lincoln, Nebraska 68524, 155ARW/MS, Attention John Hurst, 2925 NW 25th Street, Linco ln, NE 68524-1888. Delivery no later than 30 days after receipt of order. The following FARS are applicable to this order: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (lowest price embroidery machine that me ets the minimum specs provided); 52.212-3, Offeror Representations & Certifications-Commercial Items, 52.212-4, Contract Terms & Conditions-Commercial Items; 52.212-5, Contract Terms & Conditions Needed to Implement Statutes or Executive Orders-Commercial Items; 52.247-34, FOB-Destination; 52-211-6, Brand Name or Equal. Should have a minimum one year warranty on this embroidery machine. All vendors responding to this combined synopsis/solicitation must include the following in the pricing: Solicitation Num ber (DAHA25-03-T-0002), Vendor Name and Address, Vendor Phone and Fax Number, Vendor Duns & Bradstreet Number or Vendor Cage Code, and Vendor Point of Contact. Please remember that all freight charges are to be included in the overall pricing and not as a separate line item.
