- Description
- The Government has a requirement for professional Architect/ Engineering Services to provide design for the Louis Stokes Cleveland Department of Veterans Affairs Medical Center (LSC DVAMC) Project Entitled, Redesign of Roads and Parking Lots (Phase II) (inclusive of repaving) assigned our Request For Proposal (RFP) Number 541-048-03. The LSC DVAMC Project No. is 541-03-1001 and the Place of Performance if the Brecksville Facility of the LSC DVAMC located at 10000 Brecksville Road, Brecksville, Ohio. =20 THE SCHEDULED SITE VISIT DATE/TIME IS MARCH 10, 2003 AT 10:00AM; SITE VISIT MEETING LOCATION IS THE LOBBY OF BLDG. 1, LSC DVAMC, 10000 BRECKSVILLE ROAD, BRECKSVILLE, OHIO. THE GOVERNMENT TECHNICAL REPRESENTATIVES OF OUR ENGINEERING SERVICE (138-B) ARE BRIAN RICE - 440.526-3030, EXT. 6174 AND DAVID SABOL- 440.526-3030, EXT. 6165 AND THE CONTRACTING OFFICER IS LLOYD BRODNAX - 440.717-2839. Services shall include: Site investigation, preliminary plans, environmental, civil engineering, final design specifications and drawings for construction bid documents and construction period services. Submit bid documents (A-E Fee Proposal) by the date and time designated herein this Request For Proposal (RFP). The Brecksville LSCDVAMC facility has been selected for the design and subsequent construction to begin this fiscal year (2003). Preliminary plan requires the replacement and/or restoration of asphalt paving using Ohio Department of Transportation (ODOT) specifications relative to parking lots, roads, curbs, street and parking lot lighting, sidewalks, surface storm drainage system and catch basins, for use by trucks and automobiles, traffic flow, layout of temporary traffic routing and temporary signage during all phases of construction. Develop plans/estimates for review to modify entrances and exits to better accommodate feasibility of traffic flow or patterns in and out of parking lots and main entrances to the medical center. All design work shall be accomplished in accordance with VA guidelines and specifications, which are available on the Internet at http://www.va.facmgt/standards and State of Ohio Department of Transportation (ODOT) specifications and other applicable local regulations. A/E shall design work shall be in accordance with NFPA, OSHA, DER, NEC, 101 Life Safety Code, or any other state, federal or local codes and/or regulations. After selection of the A-E Firm, there will be an initial meeting of the selected A-E Firm and designated LSC DVAMC Representatives to outline the general scope and plans. =20 The following submissions are mandatory: FIRST (1ST) SUBMISSION: Preliminary study of at least two =96(2) different plans to best utilize the budgetary funds with preliminary cost estimates and preliminary drawings showing the extent and scope and traffic pattern impact for each of the two =96(2) plans; SECOND (2ND) SUBMISSION: 50% Design Drawings and specifications and cost estimates and phasing plan; THIRD (3rd) SUBMISSION: 75% Design Drawings and specifications and cost estimate and phasing plan, and FOURTH (4TH) SUBMISSION: 100% Design Drawings, specifications, cost estimate, and phasing plan.=20 The project design shall be completed within 90 calendar days (total) based upon submission approvals/documents. Submission shall include MYLAR and two =96(2) print drawings of sites showing pavements, sidewalks, curbs, storm sewers, roads and utilities, provide elevations of drainage areas that are critical. Procurement is open to both large and small businesses with award issued pursuant to the Small Business Competitiveness Demonstration Program. The NAICS Code is 541330 with the small business size standard of 4.0 million. Construction cost estimate is between $500,000.00 and $1,000,000.00. An Award will be to the A/E Firm evaluated by the LSC DVAM Evaluation Team deemed best qualified for the work. See Numbered Note 24. The following additional evaluation factors also apply: Evaluation of knowledge of healthcare facilities and prior successful experience designing projects of this size, nature, and cost; Reputation and standing firm and principal officials with respect to professional performance, general management and cooperativeness, Record of significant claims against client because of improper of incomplete architectural and engineering services; Specific experience and qualifications of personnel proposed for assignment to the project, and Record of working together as a team. An area of consideration is limited to within a 60-mle radius from the LSC DVAMC, Brecksville Facility, located at 10000 Brecksville Road, Brecksville, Ohio 44141. A subsequent award is subject to the Federal Acquisition Regulation (FAR) Clause 52.232-18, entitled Availability of Funds. THIS IS A REQUEST FOR PROPOSAL. All interested, responsible and qualified business firms within the geographic limitation desiring consideration shall submit three (3) copies of your A-E Fee Proposal to the attention of LLOYD A. BRODNAX/A&MMS, Bldg. 1, Room A-104 (541-90C(B) at the above address and your proposal must be received no later than COB, (4:30PM Local Time) on March 21, 2003. See Numbered Note 24.
