Description
NA This is a small business set-aside firm-fixed price service requirement. A copy of the Request for Quote is available as a pdf document at the following website: http://www.poh.usace.army.mil/pohct/contracting.asp. The Request for Quote will be availabl e on the issue date, March 3, 2003. Fax your quote by the closing date and time to (808) 438-8588. 1. SCOPE OF WORK: The Contractor shall furnish necessary management, labor, supervision, materials, equipment, transportation, and other incidental services to transport and relocate office furniture and equipment for four (4) offices of the U.S. Army Co rps of Engineers, Honolulu Engineer District at various locations, Hawaii. a. The Contractor shall provide boxes to the Fort Shafter Resident Office, Real Estate Division, and Environmental Branch to pack belongings and personal effects. The boxes shall be labeled in accordance with the floor plan. See paragraph 2, Schedule for specific quantities. b. The Contractor shall transport types of equipment and furniture such as but not limited to the following. See paragraph 2, Schedule for specific quanti ties. The Contractor shall be notified 3 working days prior to any changes. For example, automated equipment such as monitors, computers, mouse, keyboards, telefaxes, and printers; office furniture/appliance such as conference tables, copier equipment, e xecutive chairs, typewriters, typist stands, fans, floor mats, trashcans, sofas, microwaves, and boxes; boxes (preboxed personal effects) with dimensions of but not limited to 14 ?? x 12? x 9 ?? or 24? x 16? x 12?; and refrigerators that vary between 4.6 c ubic feet and 18 cubic feet. c. The Contractor shall provide an on-site representative to coordinate and direct all moves and serve as the point of contact with the Government. The Contractor?s employees shall be under the supervision and direction of th e Contractor. d. The Contractor shall (1) Use the hallways as staging areas. The Contractor will ensure that the building and offices remain handicap-accessible; (2) Move existing furniture, equipment, etc. to access designated items for relocation and move the existing items back to its original position; (3)The Contractor shall take the necessary precautions to ensure that existing furniture, equipment, etc. are not damaged during the move; (4) Move all items and contents therein designated for reloc ation; and (5) Use the elevator that is located in the interior, west end of Building 230. There are no elevators at Buildings T-1507 and T-223. e. The Contractor is required to submit an Accident Prevention Program Administration Plan (POD Form 248-R (Rev) 1 Mar 95 and an Activity Hazard Analysis (POD Form 184-R (Rev) 22 Apr 91) to the Government Representative for review and acceptance within two (2) days following award. The Contractor shall not perform work until the Accident Prevention Program Adm inistration Plan and Activity Hazard Analysis have been reviewed and accepted by the Government. The U.S. Army Corps of Engineers ?Safety and Health Requirements Manual,? EM 385-1-1, which describes the required safety plans and forms may be found at the following web site: http://www.usace.army.mil/inet/usace-docs/eng-manuals/em385-1-1/toc.htm. Please note that when a medical facility or physician is not accessible within five (5) minutes of an injury to a group of two (2) or more employees for the trea tment of injuries, at least two (2) employees must be first aid and cardiopulmonary resuscitation (CPR) certified from American Red Cross, its equivalent, or a licensed physician. The certificate shall not be older than three (3) years from date of issue, unless the currency is specified otherwise by the issuing agency. 2. SCHEDULE: The move shall be performed Monday through Friday between the hours of 7:30 a.m. and 4:00 p.m. Current phasing schedule is shown as follows and may be revised after award. The floor plan of the office layout will be provided 5 w orking days prior to commencement of each relocation phase. The Contractor will be notified 3 working days prior to any schedule changes. Specific quantities of office furniture, equipment, and boxes are listed but not limited to these quantities. a. 1 3 - 14 March 2003: Phase 1. Relocate the Fort Shafter Resident Office, Building T-1507, Fort Shafter Flats (which is occupied by 42 employees) to Building 127, Tripler. Relocate 500 record boxes, 37 sets of computer equipment, 50 administrative chairs, 4 scanners, 5 printers, 1 telefax machine, 1 shredder, 14 boxes of copier paper, 2 recycle racks, 22 plan racks, 1 - 7? x 20? conference room table, 22 conference chairs, 1 Television/VCR stand, 5 large plastic plants, 3 supply cabinets, 1 screen projector , 1 - 300lb design plotter in crate, 1 copier machine, 2 refrigerators, and 2 microwave ovens. All moves shall be accomplished within 2 working days. b. 20 - 21 March 2003: Phase 2. Relocate the Real Estate Division, which employs 8 people. (1) From Salt Lake Self Storage, 808 Ahua Street, Honolulu, to Building 127, Tripler. Relocate 9 file cabinets with its contents and 2 map cabinets with its contents. (2) From Room 306, Building 230, , Fort Shafter to Building 127, Tripler. Relocate 160 record boxes, 17 sets of computer equipment, 2 printers, 2 scanners, 15 trash cans, 6 desks, 13 administrative chairs, 12 conference room chairs, 2 tables, 1 copier machine, 1 refrigerator, and 1 microwave oven stand. All moves shall be accomplished within 2 wo rking days. c. 26 - 28 March 2003: Phase 3. Relocate the Logistics Management Office, Building T- 223, Fort Shafter (which is occupied by 11 employees) to Building 127, Tripler. Relocate 355 record boxes, 12 sets of computer equipment, 6 typewriters, 3 floor fans, 18 administrative chairs, 11 trash cans, 11 floor mats, 1 computer stand, 3 printers, 1 telefax machine, 1 shredder, 1 scanner, 1 refrigerator, 1 microwave oven, 4 lockers, 6 - 3? x 5? work tables, 4 storage cabinets, 1 pallet jack, 4 partiti ons, 1 computer workstation, 3 wagons, 8 - 9? x 5? racks (8 pieces per rack @ 100 pounds per piece), 2 - 5? x 5? racks (8 pieces per rack @ 100 pounds per piece), 1 scale, 4 flip charts, 6 hand truck, 1 banding machine, 2 erase boards, 1 - 50lb drill press , 1 miter saw, 1 bench grinder, 1 genie lift, 1 desk jack, 1 miter saw, 2 carts, 4 dollies, 1 - 73? x 43? erase boards, 2 - 32? x 22? erase boards, 1 - 48? x 36? cork board, 9 ladders (1 - 4? fiberglass, 1 - 10?aluminum, 2 - 6? aluminum, 2 - 6? fiberglass, 2 - 8? fiberglass, and 1 - 20? extension fiberglass), 2 waterhose carts, 2 mops, 1 bucket/ringer, 1 vaccuum, 1 dust pan, 1 - 80lb table saw, 3 - 32 gallon cans, 10 bookshelves, and 1 cardboard drum filled with sweeping compound. All moves shall be accomp lished within 3 working days. d. 3 - 4 April 2003: Phase 4. Relocate the Environmental Branch, Building 252, Fort Shafter (which employs 11 people) to Room 306, Building 230, Fort Shafter. Relocate 419 record boxes, 10 sets of computer equipment, 2 pr inters, 1 telefax machine, 1 cutting board, 1 binding machine, 1 laminator, 1 projector, 1 screen, and 1 scanner. All moves shall be accomplished within 2 working days. 3. A Government Representative will be available during the scheduled times of each move. 4. A preliminary site visit at Building 1507 is scheduled for Wednesday, March 5, 2003 at 10:00 a.m. Hawaiian Standard Time. Contractors are encouraged to attend the site visit prior to submitting a quotation. Please contact the government point- of-contact, Ms. Jeannette T.
