- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation DADA13-03-R-0016 is being issued as an unrestricted Request for Quotation. All responsible sources may submit quotes which will be considered. This solicitation document and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 2001-12. NAICS Code is 517211 and the standard size is 1500 employees. The Western Regional Contracting Office intends to award a firm fixed price contract for Pager Leasing Service for a base year plus four option years. Service area will be USA MEDDAC-AK, Bassett Army Community Hospital (BACH), Ft. Wainwright and Ft. Richardson located in Fairbanks and Anchorage, Alaska, respectively. The Contractor shall prov ide the following: 1. All tools, labor, equipment and services as necessary to provide 200 numeric and 20 alphanumeric pagers and associated extended area paging service, and; 2. Air Time: Unlimited, and; 3. Access: Internet and telephonic access for nume ric and alphanumeric pagers, and; 4. Coverage Area: Fairbanks and extended coverage area to include Eielson AFB, Ester, Fox, and Chatanika, and; Anchorage and extended coverage area to include Soldotna, Girdwood, Sutton, Wasilla, Palmer, and Willow, and; 5 . Service: All required service for local exchange number dial-up telephone service (not 800 service); Any pager requiring service must be repaired or replaced with a working pager within 30 minutes of defective/non-operational pager being presented at Con tractor?s local Fairbanks office or local Anchorage office; The Contractor is responsible for internal parts of the pager. The Government is responsible for external parts (i.e., case, belt clip, display) of the pager, and; When a pager requires repair du e to damage beyond normal wear and tear, the Contractor shall notify the Government COR as soon as the technician is aware of the cause, but prior to completion of repair(s); The Contractor shall notify the Government COR or designated representative withi n 30 minutes of any type of service failure resulting in loss of service which includes, but is not limited to, internet access. Lost pagers are the responsibility of the Government and will be paid for by the Government at fair market value, based on a no t-to-exceed price for numeric pagers and a not-to-exceed price for alphanumeric pagers, after receiving approval from the Contracting Officer. Request FOB Destination pricing. Place of performance: Fairbanks and Anchorage and extended areas as noted for Bassett Army Community Hospital, Ft. Wainwright, AK 99703. FAR 52.212-2, Evaluation ? Commercial Items apply to this acquisition. The following factors are included in 52.212-2(a) and will be used to evaluate offers: Technical Capability, Pricing, and Pas t Performance. Quoters shall provide a point of contact with phone number; submit a written statement of its technical capability as it relates to the company?s ability to provide the necessary services in accordance with the above scope of work; submit p ricing for 200 each numeric and 20 each alphanumeric pagers per year and the costs for lost pagers; and provide a minimum of two references with whom recent past performance can be verified, including a summary of the scope of work, points of contact, tele phone numbers and total contract price. The following FAR clauses and provisions apply to this acquisition: 52.212-1 Instructions to Offerors-Commercial Items without sub-paragraphs (g) and (h) as this is an RFQ; 52.212-3 Offeror Representations and Certif ications-Commercial Items; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.217-8 Option to Extend Services (notice to Contractor at least 15 days an d cannot exceed 6 months); 52.217-9 Option to Extend the Term of the Contract (notice at least 60 days with written extension 15 days prior to expiration and total cannot exceed 66 months); 52.222-48 Exemption from Application of Service Contract Act Provi sions...; 52.232-19 Availability of Funds for the Next Fiscal Year; FAR 52.252-1 Solicitation Provisions Incorporated by Reference; FAR 52.252-2 Clauses Incorporated by Reference; and, 52.212-5 Contract Terms and Conditions required to Implement Statutes o r Executive Orders-Commercial Items. The following clauses cited in FAR 52.212-5 are applicable: 52.222-3; 52.233-3; 52.203-6; 52.219-4; 52.219-8; 52.222-21; 52.222-26; 52.222-35; 52.222-36; 52.222-37; 52.225-13; 52.232-33. DFARS Clauses include 252.201-70 00 Contracting Officer?s Representative; 252.204-7004 Central Contractor Registration; 252.212-7000 Offeror Representations and Certifications-Commercial Items; and, 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Ord ers Applicable to Defense Acquisitions of Commercial Items. The following clauses cited in 252.212-7001 are applicable: FAR 52.203-3 Gratuities; 252.205-7000; 252.225-7036; 252.243-7002; 252.247-7023. All electronic and information technology (E IT) procured by this award shall meet the applicable accessibility standards at 36 CFR 1194. The contractor shall ensure all equipment is in compliance with Section 508 of the Rehabilitation Act of 1973 and submit evidence of compliance. If performance req uires the use of copyrighted or proprietary software, any proposed software license agreement shall be submitted with the offeror?s proposal. Failure to submit a proposed licensing agreement will result in the presumption that no licensing agreement is req uired or that the software proposed is wholly within the public domain. Quoters shall provide as a minimum: 1) RFQ number; 2) Name, address, and telephone number of point of contact of quoter; 3) completed copy of FAR 52.212-3 and DFARS 252.212-7000 Repres entations and Certifications; 4) completed copy of FAR 52.222-48; 5) evidence of Section 508 Compliance; 6) written statement of technical capability for evaluation; 7) two current references for past performance information; quoters are responsible for ac curate addresses and telephone numbers for submitted past performance information. 8) Prices for Base Year 01APR03 through 31MAR04 (CLINS 0001AA 200 numeric pagers and 0001AB 20 alphanumeric pagers); 1st Option Year 01APR04 through 31MAR05 (CLINS 0002AA 2 00 numeric pagers and 0002AB 20 alphanumeric pagers); 2nd Option Year 01APR05 through 31MAR06 (CLINS 0003AA 200 numeric pagers and 0003AB 20 alphanumeric pagers); 3rd Option Year 01APR06 through 31MAR07 (CLINS 0004AA 200 numeric pagers and 0004AB 20 alphan umeric pagers); 4th Option Year 01APR07 through 31MAR08 (CLINS 0005AA 200 numeric pagers and 0005AB 20 alphanumeric pagers); plus prices for lost pagers: NTE _______ for numeric and NTE _______for alphanumeric. Further, quotes must contain all commercial t erms, conditions and provisions under which the quoter conducts business in the commercial marketplace. Quoters that fail to furnish required representations or information, or reject the terms and conditions of this solicitation may be excluded from consi deration.
