Description
This is a combined synopsis/solicitation for a commercial item prepar= ed in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ is 246-03-00064. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-12. The NAICS is 421450 and the small business size is 500 employees. The procurement is set aside for small businesses. Items will be purchased by and delivery will be made to the Hunter Holmes McGuire Department of Veterans Affairs Medical Center, Richmond, Virginia. Offeror=92s proposals are hereby being requested in response to the requirements under a firm-fixed price contract and shall be submitted on offeror=92s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Quotes are to be provided for one (1) each computerized dual frequency bi-directional vascular doppler that meets the following minimal requirements: (1) must utilize a cold gas sterilizeable doppler probe (2) must have digital strip chart recorder independent of the computer report (3) must be designed to be able to run independent of the computer (4) must include a wireless remote control (5) must be operational from the front panel (6) must have a minimum of a Pentium 4, 2.0 GH, 256MB RAM, 40GB hard drive computer (7)must contain CD ROM and sound card (8) must have multi-speed computer controlled cuff inflator (9) must have the ability to export to multiple data formats including HTML, Word, DBF, and Excel (10) must contain a zip drive, floppy drive, modem, and network interface card (11) must have software utility to back-up and restore patient and customer defined settings (12) must have the ability to store unlimited names of staff using the product (13) must have the ability to customize testing sequence (14) must have at least one large storage drawer for supply storage (15) must have built in keyboard (16) multi-port inflator tubes must be accessible from either the left or right side of the machine without the need to reconfigure the machine (17) replacement probes must be available for under $150. Quote is to include all shipping and any set-up charges. Quotes shall be valid for 30 days from the offer due date. All responses must include the following: (1) descriptive literature and product number for the item offered proving the product offered meets the above requirements (2) delivery time after receipt of order (3) warranty information. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2 Service of Protest, 852.233-70 Protest Content, 852-233-71 Alternative Protest Procedure. This is a best value procurement. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/ or fax a request to 757-728-3132. Quotations and all required information must be received by 4:00pm local time March 25, 2003. Quotations shall be submitted to Barbara Simmons, Contracting Officer (90C), VA Medical Center, Hampton, Va. 23667. Solicitation number RFQ 246-03-00064 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
