- PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is being issued unrestricted as Request for Quotation No. 2003-Q-00802. The NAICS is 238290 with a size standard of $12.0 million. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-12. SPECIFICATIONS: The National Institute for Occupational Safety and Health (NIOSH) Morgantown, WV facility currently has two (2) Essco Haughton Hydraulic elevators with relay control logic. Both elevators were installed in 1970. Elevator #5 is a five stop elevator with a capability of 5000 pounds and elevator #6 is a three stop elevator with a capacity of 2500 pounds. This current project is for the contractor to provide all personnel, equipment, material, supplies, and services (except as may be expressly set forth herein as furnished by the Government) and otherwise do all things necessary to upgrade the elevator?s fire service controls to include Phase 1 and Phase 2 fire service with a primary and alternate recall floor for each elevator. NIOSH will provide the in car and hallway key switches and indicators and two contact points connected to the building fire alarm system; one for primary and one for alternate floor recall for each elevator. The contractor shall provide the remainder of the material and labor to install, including but not limited to, all controls and relays, electrical boxes, electrical wire, electrical conduit, fasteners, and control drawings for the fire service recall. NIOSH will provide a copy of the Essco controller drawing. After completion of this project, the contractor shall provide to the NIOSH project officer, one set of drawings for the additional fire service controls that were installed on each elevator. Equipment shall have a minimum of one year warranty. All work shall be completed within 60 days after award of the contract. Offerors shall identify the brand name equipment being offered and provide descriptive literature for evaluation purposes. Terms and conditions shall be clearly stated. All prices shall be F.O.B. Destination. Offers will be evaluated to determine technical compliance with the specification and past performance. The Government intends to award a purchase order resulting from this solicitation to the responsible offeror whose quote conforming to this solicitation is considered most advantageous to the Government, price and other factors considered. Your quotation must contain sufficient information to demonstrate compliance with all requirements included herein, or your offer may be rejected. The following information must be submitted with all quotations. (1) Original and one copy of a completed SF 1449, signed by an individual authorized to bind the organization; (2) Schedule of Supplies/Services (vendor quotation) and list three references of past performance on similar projects; (3) completed Representations and Certifications - Commercial Items, FAR 52.212-3 (July 2002); (4) information which clearly demonstrates compliance with the stated specifications (for evaluation purposes); and (5) acknowledgment of solicitation amendments, if any. Facsimile or electronic offers are NOT authorized. If any exceptions or assumptions are taken by a contractor with regard to any aspect of this RFQ, they should be so stated in the quote. All quotes must include a specified expiration time for the quote (60 days desired). Before the quote's specified expiration time, the Government may accept a quote, whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offers that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration. Simplified acquisition procedures will be utilized.
