Description
The Department of Veterans Affairs, San Juan VA Medical Center is issuing a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued. Written quotation will not be issued and each contractor is expected to respond based on requirements. Request for quotation, RFQ number 672-261-03 is hereby assigned to this procurement and shall be referenced on all correspondence relating to this acquisition. This RFQ is issued using the Simplified Acquisition Procedures, and incorporates all provisions and clauses as those in effect through Federal Acquisition Circular (FAC) 2001-09. Request for quote is 100% set aside for small business under SIC ____________ with a business size of _______________. The government contemplates awarding a Firm Fixed Price for =93Brand name or Equal=94. North American Industry Classification System (NAICS) 335228 will apply. The San Juan VA Medical Center has a requirement to purchase the following equipment: ( I ) Complete Pot & Pan Scullery System to include: (a) One each, Electric Tank Utensil Dishwasher, model LD-12-PT, with all it standard features, Salient Characteristics: 7-1/2 HP Stainless Steel Wash pump for 480 Volts, 3 Phase, 60 cycle, 120 gallon per minute heat steam operated, Fully automatic wash & Rinse cycles, pass thru type, 110 volt control circuit with push pad operation of =93Power on=94, =93Stop=94, =93Short, Medium, or Long Wash Cyc= les=94. Digital Display indicates =93Time Remaining=94 for each cycle, LED=92s indicates wash, rinse, and unload functions, buzzer providing time to unload, push pad as controller to displays =93Error Codes=94 for ease of troubleshooting, Running Amps of 10, Lift-up door type, Two doors, single control handle, 5=92 wall length. (b) One each, Food and waste Disposal, model C-2-L-SK-CR with control panel and all it standard features. (c) One each, Motorized Power Wash Sink, model PP-3-60-WI, 16=92 wall length, with controls and inline water heater, 60 millimeters, 480/60/3, left to right direction of operation. (d) One each, Custom Fabricated Soiled Rack Table with all it standard features, 5=92-2=94 wall length. (e) One each, Custom Fabricated Clean Rack Table with stainless steel tubular under-shelf for rack storage, 18=929=94 wall length. Required delivery date is 30 days from date of contract award. Inspection and acceptance shall be at destination. Contractor shall provide the following information: (FOB) destination pricing, lead-time, payment terms, cage code, Duns & Bradstreet number, taxpayer ID number (TIN). The following provisions and clauses are applicable: FAR 52.212-1 / Instructions to Offerors =96 Commercial Items (October 2000), FAR 52.212-3 / Offeror representation and Certifications =96 Commercial Items /July 2002 (Required for Offeror to complete and include a copy toward Contracting Officer, FAR 52.212-4 / Contract terms & Conditions =96 Commercial Items (February 2002), and FAR 52.212-5 / Contract Terms and Conditions required to implement statutes or Executive Orders =96 Commercial Items (May 2002). Commercial item applies with the following applicable clauses: FAR 52.222-26, Equal Opportunity, FAR 52.222-35, Affirmative action for handicapped workers, Employment Reports on Special Disable Veterans of the Vietnam Era. FAR 42.232-18, Availability of funds. Offerors may find the FAR provisions at http://www.arnet.gov/. The government intends to make a single award of a firm fixed price order to the responsible contractor whose offer provides among other factors, the best market value within the following criteria: (1) Overall Pricing factor, to include pondered assessment on: Installation at site, Training, Freight on Board Terms (FOB), and Warranty.
