Description
This is a combined synopsis/solicitation for commercial items prepared IAW the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation ?EMT TRAINING? is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-11. SIC Code 8049, NAICS Code 621399 (All Miscellaneous Health Practitioners). Line Item 0001: The U.S. Air Force Academy in Colorado Springs has a requirement for State and National Emergency Medical Technician Basic Training and Re-certification for 10 Firefighters prior to 30 March 03 $_______________. Line Item 0002: The U.S. Air Force Academy in Colorado Springs has a requirement for State and National Emergency Medical Technician Basic Training and Certification for 10 Firefighters $_______________. DESCRIPTION OF SERVICES: The contractor shall provide all supervision, tools, equipment, vehicles, and labor necessary to 1). Train and re-certify 10 Firefighters at the United States Air Force Academy in State and National Emergency Medical Technician Basic; and 2). Train and certify 10 Firefighters at the United States Air Force Academy in State and National Emergency Medical Technician Basic. Location of Work. Training and re-certification/certification shall be held in Fire Station #2 located at the United States Air Force Academy. Time Restrictions. Training and re-certification/certification shall be held during normal duty hours, Monday ? Friday, 0900 ? 1500. Services shall be performed in accordance with the best commercial practices. Offerors should include their DUNS, CAGE (Commercial and Government Entity) and Tax ID Numbers, and be CCR (Central Contractor Registration) and EFT (Electronic Funds Transfer) compliant. FAR 52.212-1, Instruction to Offerors ? Commercial Items, applies to this acquisition. FAR 52.212-2, Evaluation ? Commercial Items, applies to this acquisition, and the following factors shall be used to evaluate offers: Price and past performance, wherein price is considered approximately equal to past performance. FAR 52.212-3, Offerors Representations and Certifications, Commercial Items; and FAR 52.212-4, Contract Terms and Conditions, Commercial Items both apply to this acquisition with the following addenda to 52.212-4: FAR 52.237-2 and 52.242-15. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items applies to this acquisition. The following blocks are checked under FAR 52.212-5, paragraph (b): Block (1) 52.203-6, with Alt I; Block (11), 52.222-21; Block (12) 52.222-26; and Block (24) 52.232-33. The following blocks are checked under FAR 52.212-5, paragraph (c): Block (1) 52.222-41; and Block (2) 52.222-42. DFARs 252.204-7004, Required Central Contractor Registration, and DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items. Offerors must acquaint themselves with the regulation concerning Commercial Item acquisition contained at FAR 12. Offerors not in possession of the reference clauses may obtain them at http://www.farsite.hill.af.mil. Department of Labor Wage Determination 94-2079 (REV 23) for the Colorado Springs area applies.
