- CBD COMBINED/SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This Solicitation MDA94603T0024 is a Request for Quote (RFQ) for the following services: Physical Fitness Testing for Pentagon Force Protection Agency ? Defense Protective Service (DPS) Police Officers located at the Pentagon Building, Washington, DC. (iii) This Solicitation documents and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 01-12, dated January 27, 2003. (iv) The applicable North American Industry Classification System is 621111 and the corresponding size standard is $8.5M. The Small Business Competitiveness Demonstration Program does not apply to this procurement. (v) & (vi) LINE ITEMS (CLINS): CLIN 000: The contractor shall provide pre-employment Physical Fitness Testing for DPS Police Officer in accordance with the Scope of Work. Estimated annual test per year - 55 tests. SCOPE OF WORK: GENERAL: Contractor to provide pre-employment Physical Fitness Testing for Police Officers located at the Pentagon. RESPONSIBILITIES OF THE CONTRACTOR: Provide a written plan for evaluating the physical capabilities of potential Pentagon Police Officers to include but not limited to body composition, speed and agility, flexibility, muscular strength, and cardiovascular endurance. Upon approval of the written plan, provide on-call service to evaluate potential police officers as described in the Plan. Based on the evaluation of each potential officer, identify individuals who are not reasonably capable of the rigorous demands of FLET-C training or the standard functional requirements of a Pentagon Police Officer. Provide individual recommendation as to the physical/functional fitness of each potential officer and include any recommendation to correct functional limitations. Electronic recommendations are acceptable. Provide to the Administrative Officer of Defense Protective Service a copy of each evaluation and recommendations within 24 hours of each appointment. Facilitate the requested examination within five (5) workdays after being notified By the DPS Administrative Office. SECURITY REQUIREMENTS: None GOVERNMENT RESPONSIBILITIES: The Administrative Office of Defense Protective Service shall schedule appointments for each potential officer. The Government shall be responsible for all charges when a scheduled appointment is not cancelled within 24 hours prior to the appointment time. The Government does reserve the right to substitute individuals. PLACE OF PERFORMANCE: The evaluation and testing shall be accomplished at the Office of the Contractor. (vii) Date(s) and place(s) of delivery and acceptance and FOB point: Services required for the period April 1, 2003 through March 31, 2004. Services to be performed at the vendor?s site. (viii) The following provision applies to this acquisition: Federal Acquisition Regulation (FAR) 52.212-1 Instructions to Offerors ? Commercial. The following provision is being added as an Addendum: FAR 52.214-31 Facsimile Bids/Quotes. (ix) The following provision does not apply to this acquisition: FAR 52.212-2 Evaluation -- Commercial Items. The following factors will be evaluated: (a) The vendor shall provide a written plan for evaluating the physical capabilities of potential DPS Officers to include but not limited to body composition, speed and agility, flexibility, muscular strength, and cardiovascular endurance. (b) The vendor must demonstrate satisfactory past performance within the past 3 years in this area. At minimum, the following information must be provided: organization/agency, contract number contract amount, period of performance, brief description of similar service, point of contact and telephone number. (c) Pricing. All factors are of equal importance. Award will be based on the best value to the Government. (x) Vendors are to include a completed copy of provision FAR 52.212-3 Offeror Representations and Certifications -- Commercial Items along with their offer. (xi) The following provisions apply to this acquisition: FAR 52.212-4 Contract Terms and Conditions -- Commercial Items. (xii) The following clause is applicable to this acquisition: FAR 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items. The following FAR Clauses within the aforementioned clause are being selected as applicable: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-16, 52.222-41, 52.222-42, 52.222-44 and 52.232-33. Also applicable is DFAR 252.212-7001Contract Terms and Conditions Required to Implement Statues or Executive Orders Applicable to Defense Acquisition of Commercial. (xiii) Other applicable clauses/provisions: FAR 52.216-18 Ordering; FAR 52.216-19 Order Limitation; FAR 52.216-21 Requirement; DFAR 252.204-7004 Required Central Contractor (CCR) Registration.
