- Description
- This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposals (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-12. RFP 246-03-00045 should be referenced on all bids. The applicable NAICS code is 561710 with a size standard of $6 million. This procurement is a Small Business Set-Aside. The VA Medical Center, 1970 Roanoke Blvd., Salem, Virginia 24153 has a requirement for Pest Control Services. =20 This procurement is to obtain the services of a licensed Pest Control Contractor to furnish all labor, equipment and supplies necessary to perform Integrated Pest Management Services at the VAMC, Salem, Virginia, for the period April 1, 2003 up to and including March 31, 2004 with four (4) available one (1) year option renewal periods for the Environmental Management Division. Contractor shall provide the following applicable tasks for Integrated Pest Management Services for specific buildings as listed in the solicitation as Attachment 5 with the frequency as listed in that Attachment: 1) Inspection to determine which pest management measures are appropriate and required; 2) Recommending environmental sanitation practices that restricts and/or eliminates food, water, or harborage for pests via e-mail to established VISTA mailgroup for follow-up action; 3) Selection and utilization of non-chemical control methods that eliminates, excludes or repels pests (i.e. insect electrocution devices, traps, caulking, air screens, etc.); 4) Selection and use of the most environmentally sound pesticide(s) to effect control when chemical control methods are necessary; 5) Control of general structural arthropod pests (i.e. cockroaches, ants, carpet beetles, spiders, carpenter ants, carpenter bees, etc.); 6) Control of flying insect pests (i.e. house flies, stable flies, blow files, etc.); 7) Control of stored products pests (i.e. sawtooth grain beetle, red/confused flour beetles, trogoderma beetles, grain moths, etc.); 8) Control of predatory pests (i.e. bedbugs, fleas, mites, ticks, mosquitoes, etc.); 9) Control of mice and rats (i.e. house mouse, field mouse, roof rat, etc.); 10) Evaluation of control measures through follow-up inspections; Retrieval of dead roaches, etc., as a result of treatment; 12) Provision of and maintenance of all electric bug lights within all applicable buildings, maintenance to include changing out the inserts within the bug lights.=20 The Contractor shall adhere to the principles contained in the =93Program Guide 1850.2 Integrated Pest Management (IPM)=94. The Contractor shall be responsible for picking up and returning Government-Owned Havahart professional live animal traps. The Contractor shall be responsible for capture and removal of cats on VAMC premises. The Contractor shall be responsible for providing pest bird control to include relocation of pest birds. The Contractor shall provide control service for ants and winged termite swarmers as described in IPM both inside and outside buildings. Technical proposal required to be submitted which shall address the offeror=92s capability to perform the requirements of the contract; education and training, to include copies of licensure; and past performance on contracts of similar size and scope. The Contractor shall furnish to the Contracting Officer with their proposal a Certificate of Insurance which shall contain an endorsement to the effect that cancellation of, or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day written notice of cancellation or change is furnished to the Contracting Officer.=20 Provisions and clauses found at 52.212-1 Instructions to Offerors-Commercial Items; 52.212-3 Offeror Representations and Certifications =96Commercial Items; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this RFP. Contracting with Parties Listed on the OIG List of Excluded Individuals/Entities: In accordance with the Health Insurance Portability and Accountability Act and the Balanced Budget Act of 1977, the Office of Inspector General has established a list of parties and entities excluded from Federal Health Care programs. Specifically, the listed parties and entities may not receive Federal Health Care program payments due to fraud and/or abuse of the Medicare and Medicaid programs. Therefore, all offerors shall review the OIG List of Excluded Individuals/Entities on the OIG web site at www.hhs.gov/oig to ensure that the proposed candidate(s) and/or firm(s) are not listed. Offerors should note that any excluded individual or entity that submits a claim for reimbursement to a Federal health care program, or causes such a claim to be submitted, may be subject to a Civil Monetary Penalty (CMP) of $10,000 for ach item or service furnished during a period that the person or entity was excluded and may also be subject to treble damages for the amount claimed for each item or service. CMP=92s may also be imposed against health care providers and entities that employ or enter into contracts with excluded individuals or entities to provide items or services to Federal program beneficiaries. The Contractor shall adhere to Veterans Health Administration regulations and the policies, procedures, and regulations of the Medical Staff bylaws of the VAMC Salem. The Contractor shall be responsible to provide the following to its employee(s): Workers=92 Compensation Insurance; Professional Liability Insurance; Health Examinations; Income Tax Withholding; Social Security Payments.
