NA This is a combined synopsis and no other solicitation will be issued. Services to be provided per the below Performance Work Statement: WORK STATEMENT: THE COMPUTER LAB IS ONE COMPONENT OF THE INSTALLATION?S SCHOOL-AGE SERVICE. A MINIMUM OF SIXTY (60) CHILDREN PER DAY ARE ANTICIPATED TO UTILIZE THE COMPUTER LAB. SCOPE OF WORK: THE COMPUTER LAB INSTRUCTOR/TECHNICIAN (HEREAFTER REFERRED TO AS THE CONTRACTOR) SHALL IMPLEMENT THE COMPUTER LAB PROGRAM FOR SCHOOL-AGE CHILDREN, AGES FIVE (5) TO EIGTHEEN(18). THE CONTRACTOR SHALL PERFORM THE SERVICES SET FORTH IN THIS STATEMENT OF WORK. REFERENCE: DOD COMPUTER LAB, MANUAL BOOK PERIOD OF EMPLOYMENT: PERIOD OF PERFORMANCE WILL BE FROM 1 April 03 to 30 March 04. WORK SCHEDULE: CONTRACTOR SHALL WORK MONDAY THRU FRIDAY 2:00-6:00 P.M. IN ADDITION, SHALL WORK FRIDAY 2:00-6:00 P.M. AS NEEDED. DURING SUMMER MONTHS CONTRACTOR SHALL WORK APPROXIMATELY 5 HOURS DAILY BASED ON NEEDS AND REQUIREMENTS OF THE PROGRAM. CONTRACTOR: THE CONTRACTOR SHALL PROVIDE COMPUTER LAB ASSISTANCE AND TECHNICAL GUIDANCE TO SCHOOL-AGE CHILDREN ENROLLED IN CHILD AND YOUTH SERVICES PROGRMAS THE CONTRACTOR SHALL MONITOR ALL CYS CHILDREN PARTICIPATING IN THE COMPUTER LAB (CDC, SAS, MSP AND TEENS). SPECIFIC TASKS: (A) CONTRACTOR MUST HAVE EITHER APPLIED KNOWLEDGE OF MICROSOFT WINDOWS, NOVELL NETWORKS, NETOWRKING CONCEPTS AND THE MICROSOFT WINDOWS BASED APPLICATIONS OR BE A CERTIFIED NOVELL ADMINISTRATOR. (B) CONTRACTOR WILL PROVIDE GENERAL SOFTWARE EXPERTISE WITH EMPHASIS ON MATH, SCIENCE AND LANGUAGE (COMMUNICATION) SKILLS. KNOWLEDGE OF THE INTERNET (i.e. E-MAIL, WORLD WIDE WEB AND GOPHER) HELPFUL. (C) CONTRACTOR MUST BE ABLE TO CONDUCT BASIC COMPUTER SKILLS FOR STAFF WORKING IN THE CYS PROGRAMS (CDC, SAS, MSP, TEEN CENTER) (D) CONTRACTOR MUST BA ABLE TO TROUBLESHOOT PROBLEMS WITH THE STUDENT COMPUTER?S HARDWARE AND SOFTWARE, SERVER HARDWARE AND SOFTWARE AND OTHER COMPONENTS FOR THE LOCAL AREA NETWORK (LAN). THE COTNRACTOR MUST BE ABLE TO DETERMINE WHEN ADDITIONAL ASSISTANCE IS REQUIRED TO MAINTAIN THE LA B AND BE ABLE TO IDENTIFY FROM WHOM THE ASSISTANCE SHOULD COME. (E) CONTRACTOR WILL ASSIST STUDENTS IN THE COMPLETION OF DAILY TASKS, REINFORCE COMPUTER SKILLS AS REQUIRED AND POSITIVELY SUPPORT YOUTH THROUGH RECOGNITION OF THEIR EFFORTS. COTNRACTOR WILL FACILITATE COOPERATIVE LEARNING WHEN APPROPRIATE. CONTRACTOR WILL ESTABLISH CLUBS USING THE BOYS AND GIRLS CLUB AND 4-H CURRICULUM (F) CONTRACTOR WILL BE EXPECTED TO DEVELOP LINKAGES WITH PARENTS, HOMEWORK CENTER TEACHER, SCHOOL TEACHERS, VOLUNTEER TUTORS AND OTHERS IN SUPPORT OF SCHOOL AGE SERVICES, UPON WHICH THE SUCCESSFUL OPERATION OF THE COMPUTER LAB IS DEPENDENT. (G) THE CONTRACTOR WILL BE RESPONSIBLE FOR PROVIDING A QUALIFIED SUBSTITUTE INSTRUCTOR WHEN UNABLE TO INSTRUCT. THE CONTRACTOR IS RESPONSIBLE FOR PAYING ALL SUBSTITUTES. CONTRACTOR MUST NOTIFY IN ADVANCE UNABILITY TO ATTEND. MUST COORDINATE WITH SAS DIRECTOR. SUBSTITUTES MUST BE APPROVED BY THE CONTRACTING OFFICER REPRESENTATIVE (COR). THIS SUBJECT MUST BE DISCUSSED AND INITIALED BY CONTRACTOR. (H) THE CONTRACTOR WILL WORK A MINIMUM OF THREE (3) TO FOUR (4) HOURS FOLLOWING THE END OF EACH SCHOOL DAY (1430 TO 1800 HR MONDAY THRU THURSDAY. *FRIDAY WILL BE SCHEDULED AS PROGRAM NEEDS. (I) CONTRACTOR SHALL NOT LEAVE THE ROOM OR THE BUILDING UNLESS IN AN EMERGENCY OR HAS PRIOR AUTHORIZATION. FAILURE TO COMPLY WITH THIS CONDITION MAY RESULT IN THE CANCELLATION OF THIS CONTRACT. (J) CONTRACTOR WILL ARRIVE THIRTY (30) MINUTES PRIOR TO COMPUTER LAB OPENING TO ALLOW TIME FOR SET-UP. THE CONTRACTOR WILL STAY THIRTY (30) MINUTES BEYOND THE END OF THE COMPUTER LAB SESSION FOR CLEAN UP AND/OR TAKE DOWN OF THE ENVIRONMENT. COMPUTER LAB MAINTENANCE WILL BE MONITORED/SUPERVISE D BY CONTRACTOR. HE/SHE MUST MONITOR DAILY ATTENDANCE, ONLY ENROLLED CHILDREN, PARENTS AND CURRENT SAS STAFF IS ALLOWED TO PERFORM AT T HE COMPUTER LAB. PROHIBIT USE FOR UNATHORIZED PERSONNEL. (K) CONTRACTOR WILL PRESENT HIMSELF/HERSELF IN A PROFESSIONAL MANNER AT ALL TIMES: APPERANCE, DRESS AND BEHAVIOR. (L) CONTRACTOR IS PROHIBITED FROM DRINKING ALCOHOL WHILE PERFORMING SERVICES AND BLOOD ALCOHOL MUST BE ZERO (0%) PERCENT WHILE ON DUTY. CONTRACTOR IS PROHIBITED FROM SMOKING WHILE IN THE PRESENCE OF YOUTH. (M) CONTRACTOR WILL PERFORM AT FORT BUCHANAN CHILD YOUTH SERVICES, SCHOOL AGE SERVICES, MIDDLE SCHOOL PROGRAM AND TEEN CENTER. (N) CONTRACTOR WILL MAINTAIN COMMUNICATION WITH SAS DIRECTOR OR DESIGNEE. ROLE MODELS WILL BE SHOWN AT ALL TIMES. (O) CONTRACTOR WILL NOT GET PAID FOR HOLIADYS AND ON SCHOOL CLOSING UNLESS HE/SHE IS ASKED TO WORK. ? ALL THE CONTRACT CONDITIONS MUST BE READ AND INITIALED BY CONTRACTOR AND CONTRACTING REPRESENTATIVE. SECURITY: THE CONTRACTOR MUST BE SUBJECT TO BACKGROUND CLEARANCE CHECKS FOR PRIOR INSTANCES OF REPORTED MISCONDUCT INVOLVING YOUTH, ASSAULTIVE BEHAVIOR, SUBSTANCE ABUSE, LARCENY OR RELATED MISCONDUCTS SHOWN IN TABLE 2-1 OF ARMY REGUALTION 608-10. APPROVED SUBSTITUTES ARE ALSO SUBJECT TO THESE CHECKS. BACKGROUND CHECKS MUST BE COMPLETED PRIOR TO WO RKING WITH SCHOOL AGE CHILDREN. HEALTH REQUIREMENTS: CONTRACTOR WILL HAVE A MEDICAL ASSESSMENT PRIOR TO WORKING WITH SCHOOL AGE CHILDREN. THE EVALUATION WILL INCLUDE A TUBERCULIN SKIN TEST OR CHEST X-RAY. TESTING FOR SUSCEPTIBILITY TO AND VACCINATION AGAINST RUEBELLA IS STRONGLY RECOMMENDED. REQUIREMENTS FOR IMMUNIZATION AGAINST OTHER VACCINE PRE VENTABLE DISEASES WILL BE DOCUMENTED. CONFIDENTIALITY: CONTRACTOR SHALL MAINTAIN STRICT CONFIDENTIALITY REGARDING PERSONAL INFORMATION OF SCHOOL AGE CHILDREN, PARENTS AND STAFF. QUALITY CONTROL: CONTRACTOR SHALL ESTABLISH A QUALITY CONTROL PROGRAM TO ENSURE REQUIREMENTS OF THE CONTRACT ARE ACCOMPLISHED AS SPECIFIED. INITIAL PLAN SHALL BE PRESENTED TO THE COR WITHIN THIRTY (30) WORKING DAYS OF CONTRACT AWARD FOR COR INFORMATION AND APPROVAL. SUBSEQUENT MODIFICATIONS SHALL BE MADE AS REQUIRED OR TO RESPOND TO CONCERNS OF THE COR. AT A MINIMUM, QUALITY CONTROLS WILL INCLUDE THE FOLLOWING: (1) A METHOD OF OBTAINING FEEDBACK FROM SCHOOL AGE CHILDREN AND PARENTS. (2) A MEANS TO ENSURE CONFIDENTIALITY. (3) PROVIDE INFORMATION REGARDING SIGNIFICANT EVENTS OR ANY CONCERNS THAT MAY SURFACE. (4) UNABILITY TO ESTABLISH THE QUALITY CENTRAL PROGRAM CAN RESULT IN CENCELLATION OF THE CONTRACT. PHYSICAL SECURITY: CONTRACTOR SHALL ESTABLISH AND IMPLEMENT METHODS TO ENSURE THE PHYSICAL SECURITY OF U.S. ALL GOVERNMENT PROPERTY AND FACILITIES. SAFETY: CONTRACT0RS SHALL BE AWARE OF LOCAL SAFETY REQUIREMENTS AND ENSURE THAT ALL REASONABLE EFFORTS ARE MADE TO PROTECT PROPERTY FROM DAMAGE. PROFESSIONAL: CONTRACTOR MUST PARTICIPATE IN ALL CYS REQUIRED TRAININGS FOLLOWING AR 608-10. GOVERNMENT FURNISHED PROPERTY: THE GOVERNMENT WILL PROVIDE TO THE CONTRACTOR THE FOLLOWING PROPERTY FOR USE IN CONNECTION WITH THIS CONTRACT: (1) BUILDING SUFFICIENT SPACE FOR CONTRACTOR AND SCHOOL AGE CHILDREN UTILIZING THE COMPUTER LAB. (2) COMPUTER LAB FACILITY FURNISHED WITH COMPUTERS, PRINTERS, FILE SERVER, AND ALL PIECES OF SOLID LAB. (3) ONE CLASS A AND ONE CLASS C TELEPHONE AVAILABLE FOR USE WITHIN THE BUILDING. (4) HEAT, LIGHT, AND OTHER UTILITIES (5) SHARING OF OFFICE EQUIPMENT AND SUPPLIES AS NEEDED AND AVAILABLE. (6) STANDARD U.S. ARMY AND GOVERNMENT FORMS AND REGULATION AS APPLICABLE TO SCHOOL AGE CHILDREN. (7) FURNITURE TO INCLUDE TABLES, CHAIRS, ETC. THE GOVERNMENT PROPERTY MADE AVAILABLE UNDER THIS CONTRACT SHALL BE FOR THE USE ONLY IN THE PERFORMANCE OF THE CONTRACT. THE GOVERNMENT WILL NOT BE RESPONSIBLE IN ANY WAY FOR DAMAGE TO THE CONTRACTOR?S SUPPLIES, MATERIALS, EQUIPMENT, OR PERSONAL; BELONGINGS BROUGHT INTO THE BUILDING OR ON TO THE GROUNDS DUE TO FIRE, THEFT, ACCIDENT, OR OTHER DISASTER. INVENTORY OF PROPERTY WILL BE PRESCRIBED BY RELEVANT REGULATION. A COMPLETE INVENTORY OF ALL PROPERTY FURNISHED OR PURCHASED BY THE GOVERNMENT WILL BE TAKEN BY THE CONTRACTOR, WITNESSED BY THE CONTRACTING OFFICER OR HIS/HER REPRESENTATIVE, AND RECONCILED WITH THE INSTALLATION PROPERTY RESULTING FROM CONTRACTOR NEGLIGENCE OR IMPRUDENCE. CONTRACTOR FURNISHED PROPERTY: THE CONTRACTOR SHALL NOT BE REQUIRED TO FURNISH ANY PROPERTY OR MATERIALS RELEVANT TO THIS CONTRACT, BUT SHALL BE RESPONSIBLE FOR RECEIVING, CARING AND ACCOUNTING FOR AND RETURNING ANY GOVERNMENT FURNISHED PROPERTY. THE CONTRACTOR SHOULD FAMILIARIZE HIMSELF/HERSELF WITH THE PROPER CARE AND USE OF GOVERNMENT PROPERTY (AR 608-10). RECORD KEEPING: CONTRACTOR SHALL MAINTAIN RECORDS ON EACH CHILD WITH REGARD TO THEIR EFFORTS, ACCOMPLISHMENTS, AND SKILL AREAS TO BE WORKED ON. CONTRACTOR SHALL DEVELOP A MECHANISM FOR ON-GOING COMMUNICATION WITH PARENTS AND TEACHERS. CONTRACTOR SHALL MAINTAIN RECORDS TO TRACK ATTENDANCE/PARTICIPATION, SUBJECT MATTERSA NEEDS AND LINKAGES WITHIN THE COMMUNITY. CONTRACTOR WILL SUBMIT A FINAL REPORT AT THE CONCLUSION OF THE SCHOOL YEAR OR SUMMER CAMP, TO PROVIDE RECOMMENDATIONS AND ANALYSIS TO DETERMINE THE EFFECTIVENESS OF THE SERVICES THEY HAVE PROVIDED. CONTRACTOR WILL MAINTAIN RECORDS OF ALL EQUIPMENT AND ITS CONDITIONS. INVENTORY OF EQUIPMENT WILL BE PROVIDED IN JAN-MAY EACH YEAR. CONTRACTOR MUST HAVE THE ABILITY TO COMMUNICATE IN ENGLISH ORALLY AND WRITTEN. KNOWLEDGE IN SPANISH IS HELPFUL, BUT NOT MANDATORY. ---------------------------------------------- Awardee must be CCR registered.
