Description: Provide Alcohol/Substance Abuse Treatment Services to American Indians/Alaskan Natives residing in the Salt Lake City, Utah metropolitan area in accordance with the statement of work. Pursuant to P.L. 94-437, Indian Health Care Improvement Act, Title V. Award will consist of the base year plus two option years. This is a sole source requirement, see note 22.
