- Description
- RFP 528A7-15-03 is issued to provide primary care services at a location in the county of Cayuga in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to enter into a contract for community based primary/preventive care sevices for veterans. The resulting contract will be for a one year period with two options to commence on or about July 1, 2003. The Contract will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP. Service will be at an all-inclusive per visit rate for an estimated 2100 visits per year. The NAIC code is 62111 and the commercial items solicitation is unrestricted.
