- Synopsis: The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide artificial arms, legs - and repairs to same - including measurement and fittings, to Veteran Beneficiaries of the seven VA Medical Centers in Veterans Integrated Service Network 12 (VISN 12) which covers Wisconsin, the Chicago IL metropolitan area and the upper peninsula of Michigan. Pricing is based on discounts to the =93L-Code Pricing Schedule.=94 Period of performance is from May 1, 2003, through April 30, 2004, plus two, one-year optional renewal periods. This is a partial set aside for small business concerns. The VA anticipates awarding a maximum of 27 separate, indefinite quantity type contracts, with minimum, annual guarantees of $4,000 per contract. The individual Veteran Beneficiary makes the actual choice as to which contractor he/she obtains his/her artificial limbs and repairs from.=20 Electronic version and hard copy of solicitation will be available on or about March 20, 2003. This solicitation is issued under the authority of Title 38 USC 8123.
