Description
Contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to perform clinical laboratory test services for USAFA Hospital Laboratory and the Clinical Laboratory at Peterson AFB CO. The Contractor shall perform the services 24 hours, 7 days a week, 12 months per year. Daily routine courier specimen pick-up shall be provided Monday through Saturday between 3 p.m. and 8 p.m. STAT testing will be accomplished as needed and resulted to the Medical Treatment Facility (MTF) within 2 hours of request. Contractor shall provide all services and fulfill all required tasks during crisis situations. Contractor shall provide an adequate supply of dry ice and appropriate specimen containers, special instructions, special preservatives for specimen preservation, and a current list of tests with reference ranges and specimen requirements. The Contractor shall be responsible for transporting and storing specimens for at least 7 days from the completed date of testing in such a manner as to ensure the integrity of the specimen where applicable. The contractor shall provide updated information on test methodology changes (i.e. changes in reference ranges and reporting units) as soon as these procedures are put into use. Contractor shall provide and install a computer a computer terminal for the reporting of results, and assay information. Contractor shall provide a letter quality printer for providing hard copy reports. Contractor shall provide training of MTF personnel necessary to ensure proper operation of supplied equipment and retrieval of test results. All hardware and software maintenance and upgrades shall be the sole responsiblity of the contractor and shall supplied at no additional cost to the Government. Contractor shall be responsible for providing results report that reflect monthly and year-to-date data sorted either alphabetically or by the test code number. Reports shall be received by the 15th day of the month following the report period. The Contractor shall be responsible for creating, maintaining, and disposing of only those records specifically required by provisions of mandatory directives. If requested, the Contractor shall provide the original record, or a reproducible copy of any such record, within two working days of receipt of the request. Contractor shall provide published price list to USAFA Contracting Office/LGCB.
