- Description
- VISN 21 Facilities, including VA Palo Alto Health Care System, VA Medical Center San Francisco, VA Northern California Health Care System, VA Sierra Nevada Health Care System and VA Central California Health Care System requires firm to assemble and provide fifty two (52) different custom sterile surgical packs for use at the five different medical facilities. Solicitation requires offerors to submit FDA registration number as a medical device establishment and submit an affidavit that they have medical device listing for Sterile Custom Procedure Trays. Firms must have the capability to provide assembly and sterilization of the surgical packs. The solicitation is for one (1) year from date of award of contract with four (4) options to renew. Offer must be willing to ship packs through the VISN Medical Surgical Prime Vendor. NAICS Code is 423450 and size standard 100 applies. No telephone request will be accepted.
