- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The request for quote number is LAVH-04-03 The request for quote document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-12. The NAICS Code is (42183). Item 1. Three (3) each of the following items. MODEL TP-10 TWO COLUMN LIFT, MODEL TA TRUCK ADAPTERS, AND PSB POWER SUPPLY, which can be viewed by pulling the entire solicitation. All costs related to installation/freight, for solicitation, will be included in the total amount of item #1. Delivery shall be 30 days after receipt of order. FOB Destination to Unicor Federal Camp Warehouse, 8500 Doniphan Drive Anthony, TX 79821. The billing address is P.O. Box 9000 Anthony, NM 88021. Provision at FAR 52.212-2, Instructions to Offerors applies to this acquisition. Offerors are to include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications Items, with their quote. Clause 52.212-2 Evaluation-Commercial Items applies. The quotes will be evaluated based on Best Value, past-performance 50% and price and price related factors at 50%. Clause at 52.212-4 Contract Terms and Conditions Commercial Items apply. Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Items, applies, which includes 52.203-6, Restrictions on Subcontractor Sales to the Government, with Alternate I. 52.204-6 Contractor Identification Number Data Universal Numbering System (DUNS) applies. FAR 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration applies. Vendors shall submit signed and dated quotes to Unicor, Federal Prison Industries, Inc. P.O. Box 9000 Anthony, Nm 88021. If sent by Federal Express, UPS, or hand delivered, address to Unicor Federal Prison Camp Warehouse, 8500 Doniphan Drive, Anthony, TX 79821. Quotes and certifications may also be sent by facsimile at 915/886-3212 to the attention of Patricia Miranda, Contracting Officer. The date and time for receipt of quotes is March 11,2003, Mountain Standard Time. Quotes must indicate solicitation No., description of the item(s) being offered in sufficient detail to evaluate compliance with the requirements in the solicitations, terms of any expressed warranty, price and any discount terms, remit to address, a completed copy of representations and certifications at FAR 52.212-3, and acknowledgement of all amendments, if any. All proposals that fail to furnish required representations and certifications or information or reject the terms and conditions of the solicitation may be excluded from consideration. The Government may make award without discussions with Offerors; therefore, the Offeror?s initial proposal should contain Offeror?s best pricing. Any amendments hereby issued to this solicitation shall be synopsized in the same manner as this solicitation and must be acknowledged by each Offeror. Offeror must hold prices firm for 60 calendar days from the date specified for receipt of offers. Quotes received at the address specified, for the receipt of proposals after the exact time specified, may not be considered.
