Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number RFP-797-FDF4-03-0015 is issued as a Request for Proposal. The solicitation document and incorporated provisions and clauses are those in effect through FAC 01-12. The NAICS code is 541611 and the small business size standard is $6.0M. This action is set aside for small business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for HQ USAF SGXY AND HQ USAF SGR for line item 0001 Non-Personal Services, (1600 labor hours) to provide Professional Acquisition Services Support; line item 0002 local travel (maximum of 3000 miles per year; item 0003 Option Year 1, Non-Personal Services, (1600 labor hours/) to provide Professional Acquisition Services Support; line item 0004 Option Year 1, local travel (maximum of 3000 miles per year); line item 0005 Option Year 2, Non-Personal Services, (1600 labor hours/) to provide Professional Acquisition Services Support; and line item 0006 Option Year 2, local travel (maximum of 3000 miles per year). All local travel shall be conducted in accordance with the federal travel regulation. Delivery shall be fob destination to Falls Church, VA=20 22041. The successful offeror will be responsible for providing all labor, transportation, supervision, equipment, travel and expenses necessary to perform services described in the attached Statement of Work (Atch 1). The period of performance shall be from date of contract award for a period of one year and includes two (2) one (1) year option periods. The following FAR clauses apply to this acquisition. FAR clause 52.212-1, Instructions to Offerors--Commercial Items; FAR Clause 52.212-2 Evaluation=97Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions--Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification--Commercial Items; FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items; FAR Clause 52.217-8 Option to Extend Services, 52.217-9 Option to Extend the Term of the Contract; and VAAR Clause 852.270-1 Representatives of Contracting Officer. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification--Commercial Items, with the offer. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. This is a best value procurement. The following factors shall be used to evaluate offers: (1) PAST PERFORMANCE =96 Each offeror is responsible for having the attached questionnaire (Atch 2) downloaded, completed and submitted to the Contracting Officer no later than the closing date and time of this solicitation. Three (3) questionnaires from three (3) different sources must be submitted; (2) QUALIFICATIONS OF PERSONNEL =96 Offerors are responsible for submitting resumes of qualified candidates that posses the skills, knowledge and abilities of a Contract Specialist and (3) PRICE =96 For evaluation purposes, the offeror=92s total evaluated price will be the total of line items 1, and 2. Past performance and qualifications of personnel when combined are significantly more important than price. Offerors are responsible for ensuring questionnaires are sent to the Contracting Officer at the address identified in this solicitation or faxed to (301) 619-3613, Attn: Ms. Wanda Gaines. Proposals shall be evaluated based on past performance that demonstrates the same or similar service as this requirement and price. Offers are due by 4:30pm EST on April 11, 2003 at the AFMLO/VASS, 1432 Sultan Drive, Suite C, Room 101, Fort Detrick, MD=20 21702-5006 (physical address) or AFMLO/VASS 1423 Sultan Drive, Suite 200, Fort Detrick, MD 21702-5006 (mailing address).
