- ******* 9 April 03 This solicitaion has an extended response date of 5 days making the new deadline to submit packages 14 April 03 *********** This is a solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotations are being requested and a written solicitation will not be issued. Solicitation number F7380130700200 is a Request for Quotation. This solicitation document and incorporated provisions are those in affect through the Federal Acquisition Circular 2001-12. This procurement is 100% set-aside for small business concerns. The North American Industry Classification System Code is (561990). The small business size standard is $6 Million. The basic contract period is 21 Apr 03- 30 Sep 03 with four option years. The offeror shall provide as a response to this solicitation: 1) a resume detailing relevant training and experience; (2) a list of at least two recent references; and (3) a complete price list (Attachment 1), an (4) completed clause 52.212-3 (Attachment 2). All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through e-mail or mail. Regardless of method of delivery, it is the offeror?s responsibility to ensure receipt of the quotes by the time and date stated herein. All applicants will be required to be interviewed by the Senior Faith Group Chaplain. STATEMENT OF WORK & QUALIFICATIONS FOR CATHOLIC AUXILIARY CHAPLAIN: SERVICES: Non-Personal: QUALIFICATIONS: a. Must be an ordained Roman Catholic priest in good standing with the Roman Catholic Church. b. Requires a minimum of five year?s practical experience as a pastor in a military community or equivalent. Duties and Responsibilities: Individual is responsible to accomplish the following tasks: a. Perform Catholic Masses (weekend and daily), rites and liturgies. b. Brief the Wing Chaplain on issues and concerns of the Charleston AFB Catholic Community. TIME AND PLACE OF PERFORMANCE: 1. Time: Task dependent; contractor must be available to coordinate with chapel staff and be able to work a maximum of 50 units for the time period of 13 April 03-30 Sep 03. Mass is held in the afternoons on weekdays and mornings and evenings on the weekends 2. Place: All services will be performed at Charleston AFB Chapel #1 or 2. The following clauses and provisions are incorporated and will remain in full force in any resultant award. The following clauses/provisions are applicable: FAR 52.212-1 - Instructions to Offerors - Commercial Items is applicable to this solicitation. Offeror shall include with their offers a completed copy of the provision at FAR 52.212-3 - Offeror Representation and Certifications - Commercial Items. FAR clauses 52.212-4 - Contract Terms and Conditions - Commercial Items, and 52.219-6 - Total Small Business Set-Aside are applicable to this solicitation.
