Description

NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are b eing requested and a written solicitation will not be issued. The solicitation is issued as a request for quotation (RFQ). This requirement consists of the following items: A mammoscope with the following minimum specifications: Luminance range - exceed s ACR recommendations for mammography; five (5) dimming positions; Four (4) 48 inch daylight flourescent lamps per tier; 40 watts each; 4 over 4 (24 x 30 cm) 6 over 6 (18 x 24 cm) viewing format; 40 in W x 12 in H per belt viewing area; 2 single tier, h orizontal belts, 100 frames per belt capacity, total 800-24 x 30 cm and 1,200 - 18 x 24 cm film transport mechanisms; the vertical masking is one motorized shade per tier with programmable preset positions; horizontal masking is two manual shutters (left and right) per tier; Frame access is next/prior, standard buttons; Specific frame access is a numeric keypad integrated with control panel; Dimensions are shipping (WxHxD) 62x62.5x30, including casters, operating (WxHxD) 62x62.5x42 including casters, desk (WxHxD) 40x29x12; Weight - 615 lbs; Electrical requirements- 220Volt 50Hz. The contractor will also provide an autoselect function and a film jacket file. The contractor may provide a quote for a comparable equal to S&S X-Ray Products item number 08-0 0972. All prices quotes must FOB destination to include delivery costs to Landstuhl Regional Medical Center, Germany. The shipping address will be Landstuhl Regional Medical Center, CMR 402 Bldg 3739, APO AE 09180. The unit of issue is each with a quant ity of one (1). Delivery is expected within 30 calendar days of receipt of purchase order. FOB shall be destination and shipping should be included in the unit price. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this solicitation. The pro vision at 52.212-2, Evaluation -- Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: ?Lowest Price Technically Acceptable?. The contractor certifies that it can meet the delivery date by submitting a quotation. The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http:/ /www.arnet.gov/far. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. Paragraph (o) is changed to read: The contractor shall provide all standard commercial warranties to the government. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 is considered checked and is applicable to this acquisition: (11), (12), (13), (21) and (24). Only United Stated contractors registered at http://www.ccr.gov are eligible for award. All quotations for comparable equals shall provide a specification sheet.
