- The purpose of this modification is to remove statement "Service Contract Act of 1965, Department of Labor Wage Determination Number 1994-2499, Rev 18, dated 7/26/2002 is applicable" and enter statement "Wage Determination has been requested from the Department of Labor and when received will be incorporated into contract". This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is RFQ 16-08-03-ORB-2269. It is anticipated the requirement will not exceed the Simplified Acquisition Threshold. The requirement is for a fixed priced contract. The solicitation is issued as a Request for Quote (RFQ) for an Instructor at the Flatwoods Civilian Conservation Center in Coeburn, Virginia. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-12. The associated NAICS code is 611710. The small business size standard is $6.0 million. The solicitation is for one base period and two, one-year options. The Government will provide all supplies and materials needed in the performance of duties and will be available at the Flatwoods Civilian Conservation Center. DESCRIPTION OF REQUIREMENT: Please state your price per hour for base year beginning May 5, 2003 and ending June 30, 2003 with an estimated 320 hours, Option Year 1 for period beginning July 1, 2003 and ending June 30, 2004 with an estimated 2000 hours and Option Year 2 for period beginning July 1, 2004 and ending June 30, 2005 with an estimated 2000 hours. Contractor shall perform services as an Instructor on a call-when-needed basis for the Education Department at the Flatwoods Civilian Conservation Center, Coeburn, Virginia. Work will be performed on Monday through Friday from 7:30 AM to 4:00 PM. Services are to include but will not be limited to the following: (1) Teach basic education subjects, i.e. reading, math, computer skills. (2) Record and determine student progress. (3) Maintain a conductive learning atmosphere in the classroom. (4) Administer tabe testing and score. (5) Tutor low performing students. (6) Evaluate instructional materials, tests and learning aids. (7) Maintain supplies and materials for classroom use. Must have the ability to operate a computer and assist students with network searches and basic computer skills. Must have completed high school or possess a GED from an accredited institution. Contractor will be required to provide these services independently without direct supervision from the Forest Service. Contractor will provide its own insurance and benefits. PROVISIONS AND CLAUSES: The following provisions and clauses apply: 52.212-1, Instructions to Offerors-Commercial Items, 52.212-2, Evaluation-Commercial Items, applies with paragraph (a) completed as follows: Award will be made to the offeror that meets the solicitation?s criteria for price, experience and past performance. Offeror will provide documentation of qualifications and experience with solicitation package, 52.212-3, Offeror Representations and Certifications-Commercial Items (MUST BE RETURNED WITH SOLICITATION PACKAGE), and 52.212-4, Contract Terms and Conditions-Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required To Implement Statues or Executive Orders-Commercial Items, applies with the following applicable clauses for paragraph (b): 52.222-21, Prohibition of Segregated Facilities, 52.222-26, Equal Opportunity, 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, 52.222-36, Affirmative Action for Workers with Disabilities, 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, 52.232-34, Payment by Electronic Funds Transfer-Other than Central Contractor Registration, 52.222-41, Service Contract Act of 1965, and 52.222-42, Statement of Equivalent Rates for Federal Hires, fill-in information (GS-5, $11.27). The below additional contract requirement(s) or terms and conditions apply: 52.217-8, Option to Extend Services, fill in information, (within 60 days of the end of the performance period), Wage Determination has been requested from the Department of Labor and when received will be incorporated into contract and 52.217-9, Option to Extend the Term of the Contract, fill in information (30 days prior to expiration of the contract), (60 days before the contract expires), and (exercise of any options under this clause, shall not exceed three years).
