Description
SITE WALK FOR PROSPECTIVE OFFERORS IS SCHEDULED FOR FRIDAY MAY 9, 200= 3 AT 10:30 A.M. =96 MEET IN FACILITIES ENGINEERING ROOM BR 138 IN MAIN MEDICAL CENTER BUILDING 70, VAMC, ONE VETERANS DRIVE, MINNEAPOLIS, MN.=20 This is a combined synopsis/solicitation RFQ 618-102-03 for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-13. NAICS code 334419, 500 employees size standard. Solicitation is set aside 100% for small business concerns.=20 Department of Veterans Affairs Medical Center (VAMC), Minneapolis, MN is seeking urgent replacement and new =93patient wander alarm=94 technology for health care residents to eliminate use of restraints and provide safer freedom of movement; contractor to provide labor, material, and supervision/technical expertise to provide and install wander alarm systems to replace existing Accutech VP double door systems in Wards 1E, 1F, 1D, 2L, 3F and the Extended Clinical Care (ECC) and Dining Hall =96 both doors; in the best interest of economics, reuse existing system components where possible, i.e., keypad, pushbutton reset, magnetic door contacts and audible alarms; each system to include double door unit and transmit antennas, wire, miscellaneous hardware and labor to install; include patient tags with bands and tag activator/deactivator; provide labor and material to install new system in Ward 3L as above; indicate line item costs for each area/components and warranty information; Government is seeking timely install due to erratic coverage failures in acute areas =96 propose a timely installation completion period, not to exceed 60 calendar days. Contractor to provide start-up, testing, and training to Facilities Engineering and Ward/Area Personnel as needed to become cognizant of operation; provide operation and maintenance manuals. Proposals are due on May 15, 2003 by 2:30 p.m. DST in Acquisition/Contracting, Room 4G-107, VAMC.=20 Non-attendance at the May 9, 2003 site walk will not relieve the successful contractor from compliance with all field verifications and requirements to install a fully operational and functional patient wander alarm protection system. Proposals to include technical descriptive literature and past performance information to include a minimum of 3 installations (government or commercial) with that customer=92s name, telephone number and approximate dollar value. The government intends to make a best value award subject to technical and price evaluation.
