- Description
- Contract shall provide all labor, materials, transportation and supplies for IV Home Infusion Services for the VA Medical Regional Office Center, 459 Patterson Road, Honolulu, HI 96819-1522 for the period of August 1, 2003 through July 31, 2004. This is an unrestrictive commercial items procurement. The following clauses and provisions plus addendum apply: 52.212-1 Instuctions, 52.212-2 Evaluations, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders. This solicitation will be issued o/a May 12, 2003 and may be downloaded at http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa No web based offers are accepted. Questions must be in writing and may be emailed to John.Gregorio2@med.va.gov or faxed to 415/ 750-2170. Absolutely no questions will be answered via the telephone. The offeror is responsible for downloading all documents including amendments without further notice from the VA.
