- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number RFP-797-FDF5-03-0016 is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-12. The NAICS code is 812331and 812332 and the small business size standard is $12.0M. This action is unrestricted. The Air Force Medical Logistics Office/ Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for the Medical Treatment Facility (MTF) at Shaw AFB for Line item 0001 Non-Personal Services, to provide Professional Laundry Services at an estimated quantity of 6000LB per month for approximately 5 months: Line item 0002 Option Year 1, Non-Personal Services, to provide Professional Laundry Services at an estimated quantity of 6000LB per month for 12 months: Line item 0003 Option Year 2, Non-Personal Services, to provide Professional Laundry Services at an estimated quantity of 6000LB per month for 12 months: Line item 0004 Option Year 3, Non-Personal Services, to provide Professional Laundry Services at an estimated quantity of 6000LB per month for 12 months: Line item 0005 Option Year 4, Non-Personal Services, to provide Professional Laundry Services at an estimated quantity of 6000LB per month for 12 months. Delivery shall be FOB Destination to the Medical Treatment Facility at Shaw AFB South Carolina. The successful offeror will be responsible for providing all labor, transportation, supervision, equipment, travel and expenses necessary to perform services described in the attached Statement of Work (Atch 1). The period of performance shall be 1 June 03 until 30 Sept 03 and include four (4) one (1) year option periods. The following FAR Clauses apply to this acquisition. FAR Clause 52.212-1 Instructions to Offerors _ Commercial Items; FAR Clause 52.212-2 Evaluation Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions _ Commercial Items; FAR Clause 52.212-3 Offerors Representations and Certifications _ Commercial Items; FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders _ Commercial Items; FAR Clause 52.217-8 Option to Extend Services; FAR Clause 52.217-9 Option to Extend the Terms of the Contract and VAAR Clause852.270-1 Representatives of Contracting Officer. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification _Commercial with the offer. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. This is a best value procurement. The following factors will be used to evaluate offers: (1) PAST PERFORMANCE _ Each offeror is responsible for having the attached questionnaire (Atch 2) downloaded, completed and submitted to the Contracting Officer no later than the closing date and time of the solicitation. One (1) questionnaire from (3) different sources must be submitted; (2) TECHNICAL EVALUATION _ Offeror is responsible for submitting a detailed description of how requirements will be accomplished in accordance with the terms and conditions of the Statement of Work. This is to be submitted in a separate document. (3) PRICE _ For evaluation purposes, the offerors total evaluated price will be the total of line items 1 through 5. Past performance and Technical evaluation when combined are significantly more important than price. Offerors are responsible for ensuring questionnaires are sent to the Contracting Officer at the address identified in this solicitation or faxed to (301) 619-3613, Attn: Ms. Wanda Edwards. Proposals shall be evaluated based on past performance that demonstrates the same or similar service as this requirement, technical evaluation and price. Offers are due by 4:30 p.m. EST on May 27, 2003 at the AFMLO/VASS, 1432 Sultan Drive, Suite C, Room 100, Ft. Detrick, MD 21702-5006 (physical address) or AFMLO/VASS, 1423 Sultan Drive, Suite 200, Ft.
