Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. . This requirement is for delivery and disposal services of fully charged and wet batteries designed to be used in tactical and combat vehicles delivered to approximately twelve (12) maintenanc e sites located in the State of Kansas. This combined synopsis/solicitation is prepared IAW FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being req uested and a written solicitation will not be issued. This solicitation DAHA 14 R 5703 is issued as an RFP. This is a solicitation document and incorporates provisions are those in effect through Federal Acquisition Circular 2001-13. Sic code is 5013. The contract will be for a period of 1 year, with option to extend the contract by four additional one-year periods. Pricing shall be FOB Destination. A requirements type contract is anticipated. Fully charged and wet batteries designed to be used in t actical and combat vehicles consist of five (5) types: Battery 6TL, Battery 2HN, Battery 4HN, Battery group 31 and Battery group 45. Each type of battery must be priced to provide: Standard Batteries, Maintenance Free Batteries and Electrolite Free Bat teries. Written proposal shall include a FOB Destination unit price for each battery just listed. Include a statement with your proposal that the work will be accomplished in accordance with the military TM??????s and that you can meet the required deliv ery/pickup schedule. Proposal shall also include information to include agency point of contract and telephone numbers concerning your past performance with suppling these types of batteries. Maintenance Battery Sites are listed as follows: OMS #1, 200 S. Fort St, Hays, KS 67601; OMS #2, 1021 N. State St., Iola, KS 66749; OMS 3, 1201 S. McLean Blvd, Wichita, KS 67213; OMS 4, 1111 N. Severance St, Hutchinson, KS 67501; OMS 5, 16.5 Main St, Sabetha, KS 66534; OMS 7, 100 S. 20th St, Kansas City, KS 66102; O MS 8, 208 W. 17th St, Ottawa, KS 66067; OMS 9, 2810 SW Topeka Blvd, Topeka, KS 66611; OMS 11, 1709 S Airport Rd, Manhattan, KS 67502; OMS 13, 2120 First St, Dodge City, KS 67801; MATES, 741 Ray Rd, Bldg 1460, Fort Riley, KS 66442; and RSMS, Camp Funston, Bldg 1970, Fort Riley, KS 66442. The provision at FAR 52.212-3, Offeror Representations and Certifications (Jul 2002)applies to this acquisition. A completed signed copy of this provision shall be submitted with any proposal. The clause at Far 52.212-4, Contract Terms and Conditions (Feb 2002). The clause at FAR 52.212.-5, Contract terms and Conditions Required to Implement Statutes are Executive Orders-Commercial Items (Jun 1998) applies to this acquisition. Under paragraph b, the following clauses ar e incorporated: 52.203-6, Restrictions on Subcontractor Sales to the Government, with Alternate I, 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-41, Service Contract Act of 1965, as Amended; 52.222.36; Affirmative Action for Workers with Disabilities; 252.222-37, Employment Reports on Disabled Veterans; 52.225-3, Buy American Act-Supplies. The clause 52.232-33, Mandatory Information for Electronic Funds Transfer Payment (May 1999) applies to this solicitation. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes to Defense Acquisition of Commercial Items, is applicable to this acquisition.
