- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotes (RFQ) are being requested and a written solicitation will not be issued. Solicitation documents incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-13. This Procurement is unrestricted. The NAICS is 337214 with business size standard 500 employees. A firm-fixed price contract is anticipated. The South Texas Veterans Health Care System, Audie L. Murphy Division, 7400 Merton Minter Blvd., San Antonio, TX 78229 has a requirement to purchase five (5) each adjustable, mobile ergonomic dual-tier cart frame workstations, part number AFCQ1631. The workstations are 72" Wide X 36" deep which includes; frame, shelf, bumper, KA9000 Keyboard Arm Supreme, TR2000 Keyboard Tray w/Slide-out Mouse Tray, and four (4) each Flat Panel Monitor Arm AFC01 Table Mounts. The molding color is black. Additional Requirement: Five (5) each adjustable, mobile ergonomic dual-tier cart frame workstations, part number AFCQ1632, 48" Wide x 36" deep which includes; frame, shelf, bumper, KA9000 Keyboard Arm Supreme, TR2000 Keyboard Tray w/Slide-out Mouse Tray and two (2) each Flat Panel Monitor Arm AFC01 Table Mount. The molding color is black. Manufacturer is AFC Industries, Inc. Quotation shall include all warranty information. Quotations shall be submitted F.O.B. destination. All quotations are to include a completed copy of the provisions at 52.212-3, Offer Representations and Certifications- commercial Items, with quotations, (a copy of this provision may be requested by facsimile to (210) 617-5255, Attention: Lionel Johnson). The following provisions and clauses apply to this action; 52.212-4 Contract Terms and Conditions Commercial Items, 52-212-5 Contract Terms and conditions Required to Implement Statues and Executive Orders-Commercial Items Including Clauses incorporated by reference under Section b (11-16)(19)(21-22)(25), 52.212-1 Instructions to Offerors-Commercial Items, VAAR clause 852.211-77 Brand Name or Equal, 852.211-70 Requirements for Operating and Maintenance Manuals, 852-211-71 Guarantee Clause and 852.270-4 Commercial Advertising.
