- Amend the original description to read as follows: This solicitation is issued on behalf of The Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 10 for the procurement of Home Health Care Services for eligible veteran beneficiaries living within designated geographic catchment areas. Home Health Care Services shall include Skilled Home Health Care Nursing Service, Skilled Therapy Service, Bowel and Bladder Care Service, Homemaker/Home Health Aide Service, Respite Care, Hospice Care, Dietitian Services, Social Work Services, and Infusion Therapy Services. VISN 10 Geographic Catchment Areas are (1) VA Medical Center, Chillicothe, OH=20 =96 Adams, Athens, Fairfield, Fayette, Highland, Hocking, Jackson, Meigs, Pickaway, Pike, Ross, Scioto, and Vinton counties in OHIO; (2) VA Medical Center, Cincinnati, OH - Dearborn, Franklin, Ohio, and Ripley counties in INDIANA; Boone, Bracken, Campbell, Gallatin, Grant, Kenton, and Pendleton counties in KENTUCKY; and Brown, Butler, Clermont, and Hamilton counties in OHIO; (3) VA Medical Center, Cleveland, OH 44106 =96 Ashland, Belmont, Carroll, Columbiana, Cuyahoga, Erie, Geauga, Guernsey, Harrison, Holmes, Huron, Lake, Lorain, Medina, Mohoning, Noble, Portage, Richland, Stark, Summitt, Trumbull, Tuscarawas, and Wayne counties in OHIO; (4) VA Outpatient Clinic, Columbus, OH =96 Coshocton, Crawford, Delaware, Franklin, Hardin, Jefferson, Knox, Licking, Madison, Marion, Morgan, Morrow, Muskingum, Perry, Union, Washington, and Wyandot counties in OHIO; and (5) VA Medical Center, Dayton, OH =96 Wayne county in INDIANA and Allen, Auglaize, Champaign, Clark, Clinton, Darke, Greene, Logan, Mercer, Miami, Montgomery, Preble, Shelby, and Warren counties in OHIO. Request for Proposal 539-20-02 is advertised as full and open competition for the award of a single or multiple award contract(s) with NAIC 621610 and a size standard of $11.5 million. Solicitation packages are available electronically at http://www.bos.oamm.va.gov or http://www.fedbizops.gov. NO HARD COPY of the solicitation will be issued. All questions must be submitted in writing, faxed (513-475-6334) or mailed, within twenty (20) calendar days after notice announcement appears. No telephonic questions will be answered pertaining to this solicitation. In addition, no electronic offers will be accepted. Each offer should contain the offeror=92s best terms from a cost/price and technical standpoint , as contract award may be based on initial offers received without discussions. The closing date for submission of proposal will be July 15, 2003 at 2:00 p.m. EST.
