Description
This is a combined synopsis for commercial items prepared in accordan= ce with FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal RFP 528A7-23-03. This solicitation document provides incorporated clauses that are in effect through Federal Acquisition Circular 2001-14. The Syracuse VAMC has a requirement for Outpatient diagnostic Radiology services for the technical components. Procedures include but are not limited to CT, Ultrasound and open MRI. Most significant need is CT. Primary Contractor facility must be within a 10 mile radius of the Syracuse VAMC. The maximum response time required for scheduling for each modality would be no later than 3 days of calling. Intention is to schedule 2-4 CT patients per day for 45 days to decrease the backlog and reduce wait times. For all other procedures with excessive wait times, approx. 1-2 patients per week may be scheduled. All responsible sources may submit a written firm fixed price proposal with adequate information to determine professional capability and availability. Contract period will be for a 2 month period with a one month option. Evaluations will be made in the following order of importance with price being equal to 50% of total.=20 A. Professional qualifications =96 Provide resumes of only those individuals providing service under this contract. B. Availability =96 proposal will show evidence of sufficient depth to provide appointments to 2-4 VA patients per day. C. Location =96 Provide proof that office location is within 10 mile radius of Syracuse VAMC. D. Price =96 provide fixed price proposal for each procedure offered. Proposals must include enough information to adequately evaluate all criteria.=20 Incomplete proposals will be rejected. Offers shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offerors Representation and Certifications =96 Commercial items. Provisions and clauses found at FAR 52.212-1, 52.212-4, and 52.212-5 also apply. All responses must be received by 3:00 pm EST on June 6, 2003. Contracting Officer email Lisa.Tompkins@Med.VA.Gov.
